POSITION Treatment Foster Care Living Case Manager
POSITION SUMMARY The Case Manager will work with the Director in all aspects of a Treatment Foster Care program. This individual is responsible for overseeing the well-being of children who have been removed from their homes due to abuse, neglect, or other circumstances. The case manager will provide, coordinate and facilitate necessary services to foster youth and their foster caregivers to support the youth’s permanency goal.
SPECIFIC RESPONSIBILITIES include, but are not limited to: include, but are not limited to: include, but are not limited to:
Maintain necessary state relationships with licensing and other regulatory bodies, including familiarity with relevant rules and regulations.
Assist Director in recruiting prospective Treatment Foster parents and arrange training for those chosen for licensing.
Assist Director with on-going training for and evaluation of all program participants, including the meeting of training standards for licensing.
Coordinate periodic staffings whereby service providers, referring agents, and parents or significant others come together for treatment plan review and direction.
Keep appropriate records/statistics in order to facilitate and enhance program evaluation efforts.
Function on agency quality assurance committees or other special committees as assigned.
Attend regular supervisory conferences with planned agenda.
Enhance professional growth by reading, attendance at conferences, and in-service training.
Assume additional responsibilities as assigned by Director of Foster Care Services.
Coordinate the development, review, and implementation of services. Case management and behavior plans and treatments provided to foster youth.
Assists and advocates on behalf of foster youth to secure needed resources, services and supports.
Collaborates with foster parents and youth in development of crisis plans and provides crisis assistance and coaching as needed.
Assist youth with development of skills that will enhance and improve function in areas that include, but are not limited to interpersonal relationships, anger management, coping, and mental health needs.
Completion of diagnostic assessments, individual service plans and corresponding documentation as applicable and appropriate in a timely manner.
Work in conjunction with multidisciplinary team.
Conduct visits with children and foster care givers that include the ongoing assessment of child safety, permanency and well-being.
Perform case management responsibilities in accordance with state and federal laws, Oesterlen policies, procedures and council on accreditation standards.
QUALIFICATIONS
Bachelors Degree in Social Work or related field required; must be licensed or license eligible (LSW). Previous experience with children and youth in foster care preferred. Must have excellent planning and time management skills, be able to work with a wide variety of professionals and non-professionals.
