TITLE: Case Manager
Location: Springfield
RESPONSIBLE TO: Case Manager Supervisor
SUMMARY: Under the general supervision of the Regional Leadership and Case Manager Supervisor, the Case Manager is responsible for providing support and coordinating services based on client needs and treatment plans. The practice of case management is a professional and collaborative process that assesses, plans, implements, coordinates, monitors, advocates and evaluates the options and services required to meet an individual’s mental and physical health needs. It uses communication and available tools and resources to promote health, quality, and cost-effective outcomes for the clients served while promoting patient safety, quality standards of care, and professionalism in the field.
PRIMARY JOB DUTIES
Act as a liaison between therapist, client, and family of the client. Arrange family visits, coordinate transportation, ensure clients’ needs are met.
Ensure communication between family, referral source and Path Behavioral Health is appropriate and ongoing
Attend Court hearings as deemed appropriate.
Ensure documentation occurs according to policy and Path Behavioral Health standards of practice and all applicable compliance standards
Attend Supervisory and Team meetings; present case management reports that accurately represents services provided and client’s response to treatment.
Facilitate individual and group MHS/MHP as assigned.
Serve on agency quality assurance or other committees as assigned.
Assist with in-service training as directed by Case Management/MHS/MHP Supervisor
Ongoing assessment of client needs:
Assistance in achieving personal independence in managing basis needs as identified by the client and/or parent or guardian.
Facilitation of further development of daily living skills, if identified by the client and/or parent or guardian.
Maintain compliance with chart and documentation audit criteria, billing reviews and Phase System of Care requirements
COORDINATION OF THE INDIVIDUAL SUPPORT PLAN, (ISP), INCLUDING:
Services identified in the ISP
Assistance with accessing natural support systems in the community
Coordination with formal community services and systems
System monitoring
Coordination and assistance in crisis management and stabilization
Client Advocacy and outreach
Seek education and training specific to the care of clients, provide client education.
Mental Health interventions that address symptoms, behaviors, thought processes, etc., to assist clients in eliminating barriers to seeking or maintaining education and employment
Activities that increase that client’s capacity to positively impact their environment.
Assume additional duties as assigned by Case Manager Supervisor or Director.
DAY – TO – DAY RESPONSIBILITIES:
Work with the other care team members to coordinate best care for client
Resolve client/guardian concerns in a timely manner
Solve complex client issues with assigned Case Manager and/or clinician or care team
Oversee implementation of the individual plan of service, including supporting the client’s dreams, goals, and desires for optimizing independence; promoting recovery; and assisting in the development and maintenance of natural supports
Assure the participation of the client on an ongoing basis in discussions of their plans, goals, and status
Identify and address gaps in service as well as monitor under and over utilization of authorized services
Identify and assist in business development/referral source communication and collaboration efforts
Assist the client to access programs that provide financial, medical, and other assistance such as transportation services
Coordinate and assist the client in crisis intervention and discharge planning, including community supports after hospitalization
Other Duties as Assigned by the Site Lead/Regional Director/Operations
QUALIFICATIONS:
Education: High School Diploma a must. Bachelor's degree preferred
Experience/Skills:Minimum of 2 years of Case Manager experience
Physical Effort:Requires working under stressful conditions or working irregular hours.Requires sitting, viewing computer monitors and keyboarding for extended periods of times. Requires ability to communicate professional via telephone and in writing. Requires working in the community and travel
Interpersonal Skills:Requires excellent interpersonal skills and professional communication style
Background:Must pass all federal and state background checks. Educational and work history will be confirmed in compliance with company policies.
