The qualified candidate must be able to work in a fast-paced organization, enjoy patient interaction, and possess a caring attitude. Candidate will be required to assist in care planning processes and promote disease education and self-management for high-risk, chronically ill patients. Candidate will participate in the patient’s initial treatment plans, and healthcare goal development, facilitate team conferences to review treatment goals and discuss utilization of resources, and implement care plans with patients. Candidate will also monitor health information technology (HIT) tools and reports for treatment. Candidate must provide excellent customer service and patient care at all times. Must have good computer skills, electronic medical record (EMR) experience preferred.
Qualifications include the candidate preferably having a minimum of two years’ experience in a healthcare setting, but not required. Certified Diabetes Care and Education Specialist (CDCES) certification is a plus, but can also be willing to obtain certification within the first 2 years of employment. Candidate must have a current Registered Nurse licensure (RN) in the state of Missouri.
