THIS IS A NON-CIVIL SERVICE POSITION POSTING DATE: 02/28/2024 CLOSING DATE: 05/28/2024 AT 12:00AM MINIGHT SALARY: $42,213.00 - $66,960.00 Annually CLASS DESCRIPTION The Baltimore City Health Department is seeking a Grant Services Specialist II – Waiver Support Planner who will review, monitor, and process a variety of complex client cases involving the provision of social, health, vocational, rehabilitation or related services to City resident. Work of this class involves providing person centered planning to program participants. EDUCATION AND EXPERIENCE REQUIREMENTS EDUCATION: Have a bachelor’s degree from an accredited college or university in Social Work, Health & Human Services, Vocational Rehabilitation, Public Health, Nursing or a closely related health or social services field. AND EXPERIENCE: Have two years of experience coordinating, monitoring, and processing cases involving the provisions of health, social, vocational or rehabilitation services to clients. Have a minimum of two years of successful experience providing community-based case management services and /or support planning for individuals with complex medical needs. Have a minimum of two years of experience working with medical assistance programs including Managed Care Organizations. Have a minimum of two years of experience with and understanding of Medicare and private insurance programs as they relate to Medicaid. Possess the ability to work independently and within a group setting. OR NOTES(EQUIVALENCIES): Have an equivalent combination of education and experience. Strong computer literacy skills, specifically with MS Word, Excel, Outlook, and Web Based Systems. Strong understanding of and sensitivity to Baltimore City communities and experience working with diverse populations. Bi-lingual candidates are strongly encouraged to apply. KNOWLEDGE, SKILLS, AND ABILITIES Demonstrated knowledge of resources that are available for participants with disabilities, co-morbid conditions, and experiencing poverty. Experience with transitioning older adults & individuals with disabilities from Long-Term Care institutions to independent housing in the community. Demonstrated knowledge of and experience with person centered planning and self -direction. Ability to provide services in a timely, efficient, and cost-effective manner. Demonstrated experience with fee for service billing. Experience documenting case notes in a web-based system. Experience with programs and groups serving individuals with brain injury or behavioral health disabilities such as mental illness, dementia, substance use disorder, and/or other cognitive disabilities, in community-based services. Ability to be culturally sensitive in all business practices and effectively relate to the cultural/ethnic diversity of participants. NOTE: Those eligible candidates who are under final consideration for appointment to positions in this class will be required to authorize the release of criminal conviction information.
