SUMMARY: The Behavioral Health Navigator is a full-time position reporting to the Behavioral Health Manager. The ideal candidate is an organized, collaborative professional with strong planning and administrative skills. Experience and knowledge of behavioral health in an integrated setting is preferred. The BH Navigator will process all referrals for clinical counseling and psychiatry.
RESPONSIBILITY 1. Referral & Coordination of Care
- Processes all clinical counseling and psychiatric referrals. Assess patient needs and schedules appropriately with LLCHC provider.
- Connection to community mental health for patients not appropriate for LLCHC BH.
- Connection of patients to community resources to address basic social determinants of health (SDOH).
- Communicates to BH providers in a timely manner regarding patient status.
- Manages BH providers schedules. Ensures patients are scheduled for follow up appointments as recommended by provider.
- Oversees no show and cancellation appointments and contacts patients to reschedule.
RESPONSIBILITY 2. Administrative
- Provides regular updates to BH staff and attend department meetings.
- Answers telephones, screens phone calls, takes down accurate messages and provides appropriate information.
- Daily management of BH work que list.
RESPONSIBILITY 3. Other LLCHC Responsibilities
A. Attends and participates in staff meetings.
B. Maintain annual Compliatric trainings.
C. Other duties as assigned.
Qualifications
- High school diploma, associate degree preferred. Two years of work-related experience in social services administration.
- Knowledge of community mental health resources.
- Strong organizational skills and time management.
- Experience in working on an interdisciplinary team, health care setting preferred.
- Strong customer service skills.
- Ability to triage patient concerns during intake process.
- Ability to communicate effectively in both written and verbal form.
- Knowledge of computer systems, electronic medical record.
- Ability to work with a diverse population.
