Lower Lights Christian Health Center is Hiring a Clinical Quality Manager Near Columbus, OH

ABOUT LLCHC

Lower Lights Christian Health Center (LLCHC) transforms the overall health of Central Ohio, serving one individual at a time. We are focused on whole-person wellness, available to ALL in Central Ohio who need it, regardless of ability to pay! In 2019 alone, we served over 12,000 patients - with 40% being uninsured - and totaled 50,000 medical encounters!

Operating out of seven locations, we offer medical care (primary care, dental, vision, OB/GYN, telehealth), behavioral health care, 340B pharmacy, nutritional assistance programs, and more. Working hours are Monday - Friday with occasional Saturday morning coverage.

JOB SUMMARY:

The primary purpose of this position is to act as a resource, provide direction, guidance, training and supervision of the Quality Coordinators and Referral Coordinators. The CQM will be a part of the Clinical Quality team that participates in implementation of Quality Improvement Initiatives, Value Based Care Management and Population health management activities.

ESSENTIAL JOB RESPONSIBILITIES:

RESPONSIBILITY 1. Oversight of Quality and Referral Coordination Team

  • Provide supervision for Quality and Referral Coordination department teams.
  • Provide orientation/training for new Team members regarding Care Coordination, Referrals and Care Management.
  • Coordinate the daily workflow in the Quality and Referral coordination department.
  • Communicate information and or clear instructions to team members and Supervisor.
  • Assign, coordinate, track and report on tasks allocated to the Quality and Referral coordination team.
  • Provide feedback, direction, and updates on Quality/Referral coordination team/departmental goals.


RESPONSIBILITY 2. Continuous Improvement Initiatives

  • Assist and support documentation and prep for Patient Centered Medical Home (PCMH) reporting.
  • Support implementation of Quality Improvement initiatives under the direction of Chief Quality Officer (CQO)and Director of Value Base Care Management (DVCM)
  • Support and Participate in Clinical Quality and Value Based Care management functions.
  • Support the Clinical Quality Team with Provider Peer review process.
  • Support and coordinate Quality Department projects as needed.

RESPONSIBILITY 3. Care Management /Clinical Support

  • Assist Care Managers in Value Based Care Management activities.
  • Provide Care Management for high-Risk Patients as directed by the DVCM.
  • Provide input into competencies of MA, LPN and other support team members.
  • Perform Patient assessment utilizing the nursing process.
  • Document all nursing care provided according to policy.
  • Formulate plan of care for patients as needed.

RESPONSBILITY 4. Clinical Scope

  • Administer medications as prescribed by the Provider.
  • Follow approved protocols/standing orders for patient care.
  • Follow universal precautions.
  • Perform treatments, procedures as prescribed by the Provider.
  • Respond to Patient/family/caregiver needs.
  • Respond to crisis situations according to policy.
  • Utilizes Patient education for emotional and physical health maintenance.
  • Performs clinic and outreach related tasks as assigned.
  • Demonstrates effective communication skills.
  • Establishes and maintains professional working relationship with other members of the team.

RESPONSIBILITY 5. Administrative Tasks

A Maintains, sorts, and manipulates the attribution lists/reports when released so that they may be used to determine what type of care or contact is necessary for existing or potential patients.

B Aids in developing a community resource list of services available to patients and maintains collegial relationships with community resource agencies used most frequently.

C. Refers patients to other entities for education and support (Certified Diabetic Educator, Dietitian etc.) as directed by DVCM or CQO

D. Assist patient with procurement of medical supplies needed to care for themselves when necessary.

E. Completes monthly reporting /data to assist in CPC outcomes and monitoring programs.

RESPONSIBILITY 6. Other LLCHC responsibilities

  • Participate in continuing education and professional growth.
  • Maintains knowledge of, and complies with, personnel, nursing, medical, and administrative policies, and procedures.
  • Provide Clinical Administrative support in Quality and Value Based Care Management programs
  • Support and Participate in Care Management of High-risk Patients.
  • Attends all required Meetings.
  • Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, guidelines, and other State and Federal laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, CQO, or suggestion box, any suspected unethical, fraudulent, or unlawful acts or practices.
  • Performs other duties as assigned.

QUALIFICATIONS:

  • Ohio Licensed Registered Nurse
  • One -three (1-3) years’ experience in Supervision
  • Ability to work with a culturally diverse group of people.
  • Ability to function in a team environment, have good work ethics, and a mature understanding of cultural diversity.
  • EMR experience required.
  • Ability to communicate effectively in both written and verbal form, interviewing techniques, managing multiple priorities, effective time management practices, problem solving, managing change and conflict.
  • Ability to work independently and demonstrate flexibility.
  • Ability to adapt to different work environments.
  • Ability to manage time and deal with multiple priorities, problem solve, solid listening skills, relational skills, exercise sound judgment, define problems/barriers, collect data, establish facts, and draw valid conclusions to make decisions.
  • Maintain confidentiality; maintain records and data, knowledge of computer and various programs to effectively communicate as necessary.
  • Individual needs to be compassionate, competent, and demonstrate professional behavior in interactions with patients, public and staff.
  • Must be willing to work flexible hours as required by position.
