HOPE HOSPICE is Hiring a Medical Records Specialist Near New, TX

MEDICAL RECORDS SPECIALIST
Job Summary: As a member of the Medical Records Team, assists in the compilation and maintenance of accurate and complete written and electronic patient medical records.
Job Status: Full-time; Nonexempt
Qualifications:
  • High school diploma or equivalent. Minimum one-year general office experience, preferably in the medical field.
  • Knowledge of medical terminology preferred
  • Excellent computer skills required (Microsoft Office and Outlook data entry/scanning)
  • Must have excellent written, organizational, telephone, and communication skills
  • Must be detail oriented in addition to strong problem-solving skills.
  • Able to work effectively within a team approach as well as independently.
DUTIES & RESPONSIBILITES
A. Medical Record Compilation
  • Compiles and maintains current, accurate, and complete patient written and electronic medical records.
  • Collects and sort mailroom faxes and distributes to appropriate recipients.
  • Processes physician orders including checking for accuracy, completeness, signatures, communicating additions/revisions to clinicians, logging status into electronic data base and following up with outstanding orders; all per agency policy.
  • Collects, sort and files documentation in patients’ medical records.
  • Archives written patient records and makes arrangements for off-site storage and maintains accurate log of stored patient records. Arranges for shredding of paper records per time standards for Medicare, Medicaid and private insurance requirements.
  • Responds to medical record requests from patients, families, insurance companies, etc. following the HIPAA guidelines.
B. Data Entry
  • Enters and submits Medicaid information to appropriate website for all eligible patients.
  • Performs verification of physician licensure and restriction status. Communicates any noted restrictions to Director of Patient Care Services.
  • Enters information and revisions to location and physician maintenance lists in electronic record software (as a backup to intake except for updating licenses which is the responsibility of medical record department)
  • Monitors and corrects death/discharge files for CAHPS surveys.
  • 1st of each month runs report for previous month’s deaths and discharges
C. Medical Record Audit
  • Admission audit for technical components
  • Live discharge audit for correct forms completed.
  • Other technical audits as directed
D. Communication/Documentation Reporting and Support-
  • Ensures admission orders, certifications, re-certifications, and Medicaid re-certifications are delivered to appropriate IDT for timelines.
  • Send discharge summaries to attending physicians.
  • On a weekly basis, compiles and distributes list of patients requiring recertification within the next 30 days.
  • Assists receptionist in answering overflow calls.
  • Prints for distribution by the RNCM to appropriate facilities (and logs):
    • On admission: consents, face sheet, care plan by discipline, medication sheet, nurse admission assessment, initial Plan of Care/CTI
    • Recerts: Plan of Care/CTI
  • On admission, prints Plan of Care with medication list for home patients.
  • Complies with requests for release of Protected Health Information as approved
  • Notarizes documents for agency
  • Weekly print required IDT documents following meeting days.
  • Prints nurse notes for patients living in facilities for RNCM/alternate to take to facilities for their records.
GENERAL
  • Support philosophy and objectives of Hope Hospice.
  • Maintain professional appearance, behavior, attitude, and integrity.
  • Keep abreast of and abides by policies and procedures of the agency.
  • Maintain confidentiality and ethical behavior.
  • Promote a team approach through cooperation, mutual respect, flexibility, and dependability.
  • Participate in appropriate meetings and activities, including Quality Assessment/Performance Improvement.
  • Exhibit good stewardship of resources, including self-care.
  • Read materials and attend in-services, seminars, or conventions for professional growth.
  • Contribute to an atmosphere of excellence, mutual respect, and caring.
  • Perform other duties as assigned.
Working Conditions: Works in a routine office environment. Prolonged sitting and some standing required. Occasional need to lift, pull, carry and push items weighing up to fifty pounds. Frequent need to stoop, kneel, and reach while accessing files. Requires working under some stressful conditions to meet deadlines and staff/patient needs. Requires hand-eye coordination and manual dexterity. Required problem solving skills.
