HCP Talent is Hiring a Social Worker Near Bronx, NY
- Responsible for 1 sub-acute unit and 1 long term care unit (80 beds in total)
- Ensure that all Social Work documentation is completed in accordance with facility policy and procedure as applicable
- Coordination of Interdisciplinary Discharge Planning (Knowledge of community resources
- Demonstrate compliance with government and accrediting agency standards and regulations pertaining to Social Work
- Interface with facility departments and outside agencies in order to provide a complete plan of care and to facilitate services.
- Liaison between family and other interdisciplinary team members
- Day shift (7am-3pm) Monday-Friday
- Competitive Compensation: $70K per year
