We are currently seeking a Social Worker to join our team. 

Discharge planning for those in our sub-acute program is a critical focus of the position as well as assisting with adjustment issues related to changing life circumstances and serving as an advocate for residents and their families. Collaboration with an interdisciplinary team of medical professionals is a daily occurrence and one that is an enriching and challenging experience.

Qualified applicant will be a motivated self-starter with a degree in Social Work. 

Job Responsibilities:

• Work as part of an interdisciplinary team to provide comprehensive discharge planning, provide emotional support to patients/residents and families, complete MDS and psychosocial assessments.
• Coordinate and facilitate discharges, care planning and completion of all documentation
• Facilitate team and family meetings, education on resident rights and well-being needs of the vulnerable elderly and ill population..

Education: Bachelor's Degree in Social Work at a school accredited by the Council on Social Work education Preferred

Experience: Minimum to At least one year experience in a health setting or with the aged is desirable. Must be able to Work well with disabled, elderly, emotionally upset, and at times hostile people in the community. Must possess excellent customer service skills in dealing with residents, staff, community, Agencies, and all contacts with the facility. Must be professionally motivated and ethically. Focused according to Social work standards and practice. 

Certifications: Must have Bachelor’s Degree in Social Work or some related field.

Physical Requirements: Must be able to hear with corrective or adaptive devices and see adequately with corrective Eyewear. Must be able to stand for long periods of time, bend at knees and waist, push and Pull, turn and lift a minimum of 50lbs.
