Job Description
Position Summary:

The Social Worker will work with the program participants, ages 18-24, providing support and counseling to improve the youth and young adult's life.

Essential Job Functions :

• Provide one-to-one counseling as needed as well as group counseling sessions
• Maintain regular contact with participants to meet program standards and achieve program retention
• Ensure that referrals are made to appropriate internal and external services as required to meet the needs of the participant and their families
• Write progress notes on sessions with program participants
• Meet with program participants in group and one-on-one
• Conduct thorough assessment, develop a psychosocial that informs the Individual Service Strategy (ISS), and assist with development of educational and career-related goals collaboratively with participants based on motivational interviewing process
• Maintain open communication with program participants and program graduates through face-to-face contacts, phone contact, social media such as Facebook
• Communicate regularly with partners who refer and/or supervise participants.
• Provide outcomes reporting
• Maintain participant files, including progress notes and report on progress toward goals
• Provide follow-up services for program graduates
• Provide weekly reports to program director
• Participate in regularly scheduled supervision and on-going training
• Attend meetings and participate in professional development training offered

Education and Experience :

• Licensed Master's Degree in Social Work (LMSW or LCSW).
• Two (2) years of experience providing social work/mental health services to youth and young adults, including one year of experience with disconnect youth and young adults within the last five years.
• Experience working within a youth development model a plus
• Ability to evaluate and intervene in crisis situations
• Bilingual in Spanish a plus.
