Shift: Days * 3 positions *
Hours Per Week: 40 hours
- Provides assigned patient counseling services and case management.
- Systematically and continually assesses the patient's coping mechanisms and progress.
- Utilizes appropriate teaching methods and/or referral sources.
- Coordinates patient's discharge planning needs and readiness for discharge with other healthcare members.
- Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment.
- Follows Standard Precautions using personal protective equipment as required.
Licensure / Certification / Registration:•Required Credential(s): • Counselor specializing in Alcohol and Drug Abuse credentialed from the New York State Office ofAlcoholism and Substance Abuse Services obtained prior to hire date or job transfer date.
Education :High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degreerequired.
