Registered Nurse (RN) Case Manager - Home Care: Enjoy independence and flexibility in the field to care for patients in their home environment.
Hours: Full-time | Days | Monday - Friday | Holiday rotation (work two holidays per year) | Weekend rotation may apply dependent upon staffing levels.

Annual Salary: $89,148.80 - $101,233.60

Scrubs Allowance
Territory: Our office is in Roseville, MN and the RN will have a general territory within our service area where they will provide in-home care to patients. The general territory for this position would primarily be in the Northwest section of the Twin Cities Metro area (i.e., Cities would include those that are South of Highway 394, and West of 35W).

Be a part of a collaborative and supportive interdisciplinary team approach to care.
Flexible scheduling of patient visits.
A few must-haves for this position:
Graduate of an accredited school of nursing and has a current license as a Registered Nurse in the State of MN.
Able to travel within a geographic area serviced by the office. Reliable transportation, valid driver's license, and proof of auto insurance.
CPR Certification
Two (2) years of RN experience.
One (1) year case management experience preferred.
Community health and a wide range of Medical-Surgical experience preferred.
Some functions of what you would do as an RN Case Manager:
Provides services that are ordered by the physician as indicated in the plan of care.
Completes nursing initial and ongoing assessments of all in home clients within specified deadlines.
Participates in ongoing interdisciplinary assessment of the patient.
Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
Visits in home clients to evaluate the quality of care being provided and review clinical notes.
