Essential Functions
Provides case management services that assess and meet client needs in such areas as emotional support, medical assistance, emergency food or welfare. This includes helping the client to set goals and a plan for meeting their needs and then assessing progress towards meeting those goals. Maximum case load 15 individuals.
Coordinate with other community-based organizations to see that client has access to applicable human service resources such as clean; safe and affordable housing, spiritual or religious support, employment or employment readiness training; chemical health treatment, and legal or medical services.
Facilitate monthly in-house support group designed to assist individuals in advancing their interpersonal social and basic life skills. This will include relationship building, financial literacy, independent living, nutrition and food preparation, house cleaning, educational achievement and parenting skills. All of these areas are designed to support the individuals’ efforts to become independent and self-sufficient contributors.
Complies and keeps accurate records of statistical data to meet agency and contract requirements.
Documents client goals, plans progress, concerns, or activities into their case records.
Prepare and submit monthly program progress report to director as well as the annual and semi-annual reports due.
Ensure assigned property is maintained and up to code. Report all repair needs to Director.
Monitor weekly UAs with housing clients.
Coordinate with County and State representatives to ensure eligibility requirements for all new and current participants are met.
