Job Summary:
We are currently seeking a motivated and talented individual to join our team as a Case Manager. This position offers an exciting opportunity to contribute to our company's success and grow professionally. As our Case Manager, you will be responsible for being part of our whole-person care approach by being the first and last person our patients will interact with. Beginning with onboarding new patients, to behavioral health check-ins, and closing out patients as their sessions come to an end. We will also have you cover at the front desk to help support staff, by answering phones, emails, and text messages a couple of times a week (every Thursday from 4p-8pm and one Saturday a month from 830am-130pm). This is a hybrid position, with part of the hours working from home.

Responsibilities:
- Perform patient onboarding services in a timely and efficient manner
- Collaborate with team members and providers
- Maintain a high level of quality and attention to detail in all work
- Follow company policies and procedures to ensure compliance
- Continuously seek opportunities for process improvement and efficiency
- Communicate effectively with team members and stakeholders

Experience:
- 2 years of experience in a similar role or industry preferred
- Strong knowledge of G-Suite
- Excellent problem-solving and analytical skills
- Ability to work independently and as part of a team
- Effective communication and interpersonal skills
