POSITION OVERVIEW

The Housing Case Manager will provide Housing Stabilization Services (HSS) to assist people with disabilities and a history of homelessness to live successfully in the community. The purpose of HSS is to support an individual’s transition into housing, increase long-term stability, and avoid future episodes of homelessness or institutionalization by providing long-term support. The services provided will emphasize personal goal setting, developing strategies to achieve goals, skill-building, client advocacy, connections to resources, and overcoming unexpected client issues.

ESSENTIAL DUTIES, FUNCTIONS, AND ACTIVITIES

  • Develop a Housing-Focused Person-Centered Plan for clients.
  • Provide onsite HSS for clients living at our site-based supportive housing locations.
  • Meet with clients 1-4 hours a week in the community, the client’s home, or the company's office.
  • Complete an initial housing assessment, create a goal plan, and complete case notes.
  • Identify and assist with addressing barriers to maintaining housing, including crisis intervention.
  • Help clients to understand and maintain lease compliance. Ensure the living arrangement is appropriate and safe.
  • Assist clients to identify community resources, purchase household goods, and obtain food.
  • Support people to maintain housing through prevention and early identification of behaviors that may jeopardize their continued housing. Refer clients to appropriate resources.
  • Assist with the housing recertification process for rental assistance.
  • Train clients how to be a good tenant, maintain lease compliance, and manage conflicts.
  • Support clients to understand and increase/maintain the income and benefits necessary to retain housing. Support the person in applying for all benefits (county, state, tribal and federal) to help them afford their housing.
  • Coach clients to build natural housing supports, services, and resources within the community.
  • Participate in daily morning meetings with the landlord.
  • Provide clients with transportation to appointments, supports, and services related to health and housing.
  • Maintain a caseload, meet productivity standards (26 hours per week), and proactively promote Anishinaabe Endaad services across professional networks.
  • Maintain timely documentation of case notes and participate in internal note audits.
  • Participate and assist in the tasks associated with client care, including evaluating client safety and appropriate interventions related to their functioning.
  • Act as an advocate on behalf of clients with the landlord, professionals, and the broader community.
  • Provide crisis assistance as needed.
  • Perform other duties as assigned.

QUALIFICATIONS

Experience: Minimum of 1-year case management experience required; and

Education: High school diploma or equivalent; and

License/Certification: Clean driving record, valid driver’s license, reliable transportation, and vehicle insurance; and

Complete all mandatory DHS trainings within 30 days of hire.

KNOWLEDGE, SKILLS, AND ABILITIES

  • Knowledge of established concepts, principles, and practices related to housing, homelessness, and harm reduction.
  • Advanced understanding of American Indian cultural practices.
  • Ability to use critical thinking skills, function independently, and make complex decisions based on care standards, policies and procedures, and supervisors instruction.
  • Experience with writing case notes and maintaining proper documentation.
  • Knowledge of various drug classes (both licit and illicit), their benefits, side effects, and adverse reactions.
  • Ability to prioritize tasks, organize time effectively, and exceed deadlines.
  • Must have good customer service skills to foster a patient-centered approach.
  • Excellent interpersonal skills in handling interactions with staff, other agencies, groups, and patients and families; capable of working with and actively participating with a diverse team.
  • Basic knowledge of principles, procedures, techniques, trends, and literature of social work (e.g., mental disorders, human development, social policy, developmental disabilities, cultural factors, etc.).
  • Ability to handle sensitive situations in a calm, professional manner.
  • Previous experience with using electronic health records is required. Experience using Procentive is preferred.
  • Strong observation, assessment, and crisis management skills.
  • Ability to handle stress, adapt when issues come up, and work on the go.
  • Ability to work independently, with minimal direction, and within a team.
  • Basic knowledge of current standards in mental health, housing/homelessness, public health, and Federal, State, and Tribal programs.
  • Knowledge of substance abuse and causes and symptoms of mental health.
  • Knowledge of Google Drive, Gmail, Microsoft Office Suite (Word, Excel, etc.), internet software, and appropriate storage of electronic files.
  • Ability to perform other duties as assigned.

WORK ENVIRONMENT

Work environment: The work environment characteristics described here represent those employees encounter while performing the primary functions of the job. Normal office conditions exist, and the work environment’s noise level can vary from low to moderate. Regular travel between program locations and transporting clients is required. This position could be exposed to certain health risks inherent when providing in-home services, such as potential exposure to animals, sharps, and infectious bodily fluids.

Physical demands: The physical demands described here represent those that must be met by an employee to perform the primary functions of the job successfully. While performing the job’s duties, the employee may frequently stand, walk, sit, bend, twist, talk, hear, and perform repetitive motions. There may be prolonged periods of sitting, keyboarding, reading, and driving or riding in vehicles. The employee may occasionally lift and move up to 50 pounds. Specific vision abilities required by this job include reading and using computers. Talking and hearing are essential to communicate with the community, visitors, employees, neighbors, landlords, professionals, and vendors.

Mental demands: There are many deadlines associated with this position. The employee must handle frequent interruptions, multi-task, and interact with a wide variety of people on various and, at times, complicated issues.

Confidentiality: Employees must comply with HIPAA and other data privacy standards.

Background Study: This position is subject to a DHS NetStudy 2.0 background check.
