AFTERCARE TRANSITIONAL SERVICES and EXTENDED FOSTER CARE

CASE MANAGER JOB DESCRIPTION

GENERAL DESCRIPTION: Rebound, Inc. provides a variety of services to youth and families in the Twin Cities, with a concentration on North Minneapolis and the West Side of St. Paul.

Rebound’s Extended Foster Care ("EFC") program supports young people who have or will age out of foster care benefits to sign up and remain eligible for EFC benefits.

Rebound's After Care Transitional Support Program (“ATS”) is a two-year, drug and alcohol-free, transitional support program for 18 – 24-year olds with a history of juvenile justice involvement. ATS is committed to supporting participants in being successful in the community. To this end, the following services are provided:

Monthly resources

  • 2-year housing/rent/utilities subsidy. Subsidy is based on your income.
  • Free monthly activities
  • Furniture and other household items
  • Support with transportations, and other barriers to success

MINIMUM QUALIFICATIONS:

  • Bachelor's degree in Corrections, Criminal Justice, Sociology, Psychology, Social Work, Education or related behavioral science field AND one-year PROFESSIONAL experience in corrections, human services or related behavioral science field

OR

  • Two years full time PARA-PROFESSIONAL experience in corrections, human services or related behavioral science field

Required license(s):

  • Valid driver's license required
  • First Aid/CPR certificate within 6 months of hire.

PREFERRED QUALIFICATIONS: Social Work license

DUTIES & RESPONSIBILITIES: These examples do not include all possible tasks in this work and do not limit the assignment of related tasks in any position of this classification. Regular attendance according to the position's management approved work schedule is required for all positions.

  • Effectively engage youth prior to discharge from foster care or after discharge from foster care, to aid youth in effectively advocating for their extended foster care subsidies, and support them as needed.
  • Process referrals for EFC and ATS in a timely manner.
  • Construct and implement individualized goal plan for each youth based on assessed strengths and needs.
  • Identify and connect participants to community resources which support their individualized goal plan.
  • Meet with youth as necessary to support individualized goal plan.
  • Evaluate, document, and monitor progress toward goals and objectives.
  • Facilitate activities.
  • Provide immediate response in crisis situations.
  • Provide a written notes and updates for individual youth as needed.
  • Provide feedback to county and community partners as needed.
  • Assist youth with apartment application process
  • Effectively communicate with county workers and our partnered landlords
  • Work with partner landlords to place youth in supported housing.
  • Monitor youth for program compliance (rent payments, rule following, engagement, etc).

KNOWLEDGE, SKILLS & ABILITIES AND WORK ENVIRONMENT:

  • Knowledge of human development, family dynamics and systems, including crisis management and cultural competency.
  • Knowledge of community resources and funding sources.
  • Comprehensive knowledge of computer software and information systems.
  • Interviewing, negotiation, and listening skills.
  • Ability to communicate effectively both orally and in writing.
  • Knowledge of evidence-based practices including assessment, case planning, and cognitive interventions.
  • Knowledge of criminal behavior and/or juvenile delinquency.
  • Knowledge of basic psychology.
  • Knowledge of the criminal justice system in Minnesota; working knowledge of the functions of its component parts.
  • Knowledge of statutes, rules, and policies pertaining to foster care and the juvenile justice system, and the ability to research the same.
  • Knowledge of the child protection and foster care systems in Minnesota
  • Ability to make decisions under stress, while abiding by rules, laws, policies and procedures.
  • Considerable ability to work effectively with young people, landlords, department and County associates, and the public in a professional manner.
  • Ability to work alone and in a team to accomplish tasks, plan and resolve problems.
  • Ability to communicate effectively both orally and in writing.
  • Ability to apply departmental policies and procedures to assigned duties.
  • Demonstrated ability to resolve conflict.

WORK ENVIRONMENT: Case Managers are expected to work at all of Rebound’s facilities including group homes and admin office. Case Managers must also feel comfortable doing home visits, helping youth move into housing, taking young people into the community, and visiting them in facilities.

PAY: $48,000.00 - $52,000* per year

*Depending on education level
