Job Title: Floating Case Manager - Community Health

Schedule: Full Time or Part-Time, minimum requirement of 2days per week and 1 weekend per month.

Compensation: Up to $130,000 per year.

This dynamic position caters to professionals fromhospital, outpatient, or clinic settings, looking to supplement their work withenriching and impactful patient interactions. As a Floating Nurse, youwill have the unique advantage of providing personalized care to patients inthe comfort of their homes, offering a level of autonomy and connection thatgoes beyond the confines of a single facility.

Floating NurseResponsibilities:

• Helping with company growth and being flexible on drivinginto areas we have overflow clients.
• Administer prescribed medications accurately and safely,ensuring proper dosages and adherence to medication schedules.
• Provide specialized wound care and treatment to patientswith acute or chronic wounds. Assess wound healing progress, apply appropriatedressings, and implement infection prevention measures to facilitate optimalrecovery and prevent complications.
• Empower patients and their families through healtheducation, offering guidance on managing chronic conditions, promoting healthylifestyle choices, and addressing potential risks.
• Maintain accurate and detailed records of patientsassessments, care plans, and interventions. Ensure timely and effectivecommunication with the healthcare team, including physicians, therapists, andother professionals, to facilitate coordinated care and continuity oftreatment.

Floating NurseQualifications:

• Active nursing license in the state of Minnesota
• Valid Driver's License and ability to drive 50 miles fromhome.
• Clinical experience, preferably in home care or relatedfields.
• Strong assessment and care planning skills.
• Excellent communication and empathy.
• Effective time management and organizational abilities.
