Overview – Service Coordinator

We have excellent career opportunities for individuals looking to join an innovative and diverse Property Management team. We are seeking qualified candidates for the position of Service Coordinator. The Service Coordinator will strive to improve the quality of housing offered by CPO Management Services for residents by helping them be aware of access to resources they may need/want, in ways that maintain dignity and autonomy. A successful candidate will possess professionalism, including the highest levels of customer service, strict confidentiality, follow-through on commitments, and effective communication skills.

Responsibilities

The Service Coordinator duties include, but are not limited to:

  • Analyzing resident needs.
  • Researching and establishing linkages with service providers.
  • Educating residents, families and staff of the services that may be necessary to assist residents with self-reliant lifestyles.
  • Serving as resident advocate and liaison for services/resources.
  • Fostering a sense of community.
  • Tracking and reporting progress on service coordination activities.
  • Investigating potential funding sources to benefit residents and share with grant manager.
  • Performing other duties as assigned.

Qualifications

Minimum Qualifications:

  • A Bachelor of Social Work and or Human Development & Family Sciences with a specialization in Gerontology or degree in Psychology or Counseling preferred. Along with 2-3 years of experience in social service delivery with an elderly and/or family population

AND

· Valid driver's license and insurance

Selected candidates will be subject to the following background checks:

· Criminal History*

· Drug Screen

· Motor Screen

*The final candidate selected for the position will be required to undergo a criminal background check. Criminal convictions do not necessarily preclude an applicant from consideration for a position. An individual assessment of an applicant’s prior criminal convictions will be made before excluding an applicant from consideration.
