Description
  • Thinks, communicates and behaves as a cause-driven leader and role model for other staff and members when it comes to desired staff behaviors; promotes youth development, healthy living, and social responsibility in all job-related functions.
  • Engage and enroll families in Ohio's Early Intervention program.
    • Gathers information from parents after receiving referral from Central Coordinating Site.
    • Conducts interviews and home visits with referred families and secures reports and other pertinent information from agencies and professionals involved in delivering services to the family/child.
    • Participates in developmental evaluations/functional assessment.
    • Summarizes both in written and verbal form, all pertinent information significant to the determination of need for early intervention services.
    • Conducts history-taking, evaluates strengths and needs of families and refers infants, toddlers, and families to appropriate agencies.
    • Directs referrals to qualified professionals and agencies providing early intervention services, use knowledge of resource to help parents decide which of the appropriate services they wish to utilize.
  • Assures completion of IFSP and high quality outcomes that are functional and meet the 6-step criteria as prescribed by the Ohio Department of Developmental Disabilities (DODD).
    • Initiates the Individualized Family Service Planning (IFSP) processes with family and other agencies involved in serving the family and child.
    • Encourages and fosters the family's independence in decision making about the services that they are receiving, and creating goals for the family and child.
    • Complete IFSP's and outcomes to ensure utilization of specific DODD strategies.
    • Complete and use an accurate Child Outcome Summary (COS) statement, strengths, and needs for each of the three outcomes.
    • Completes IFSP Reviews in a timely manner, no less than every 180 days.
  • Acts as a facilitator and convener with community partners including Local School Districts, FCBDD, and others, to ensure policies and procedures meet families' needs and are followed as intended.
    • Initiates the transition process from EI services to preschool services. Collaborates with public school districts special needs preschool team and other appropriate transition agencies.
    • Attend weekly FCBDD team meetings and be prepared to present each applicable child on their caseload to the team.
    • Complete monthly contact with individual families within the policy guidelines.
    • Participate as part of the ECRN Team at community events, program related services and YMCA activities.
    • Suggest and be part of solutions to challenges that face the staff and ECRN Program.
    • Initiate planning with other service providers working with the family.
    • Develop and facilitate good rapport with community service representatives.
  • Maintains 100% compliance in meeting the three (3) major compliance areas as mandated by federal, state, and county requirements: 45 day timeline, timely receipt of services, and transition components.
    • Complete monthly files review summary sheets and turn into Clinical Manager for a full file review, TRS, 45 day, or TPC.
    • Review the Service Coordinator caseload report bi-weekly to monitor 45 Day compliance for children who have been determined eligible and those without eligibility.
    • Review the Timely Receipt of Services (TRS) CAP log during bi-weekly supervision to monitor TRS deadlines.
    • Review the Transition Compliance (TPC) report every month in order make sure that TPCs are being scheduled and/or carried out in a timely manner.
    • Review the EI Redetermination Report and internal ECRN Annual IFSP spreadsheet to monitor upcoming annual IFSPs reassessments and redetermination deadlines.
  • Maintains thoroughness and accuracy of all charts and data.
    • Ensure all client files are accurate and complete and all children are being billed to the appropriate funding source.
    • Ensure data is entered into the Early Intervention Data System (EIDS) within five (5) business days.
    • Review data collection "Sunshine Sheet" sent each Monday by the county and provide updates to the county within two business days.
    • Complete accurate and up to date case notes for each file on a weekly basis.
    • Gathers appropriate information from referral in a systematic and timely manner. Works in conjunction with referral source to obtain information regarding the individual being referred, informs them of the process of the individual through the intake process, and educates the referral sources regarding the intake and evaluation processes.
  • Carry ongoing open caseload of at least 55 children and manage approximately 10-15 referrals at any given time.
    • Screen children utilizing the ASQ and ASQ-SE as appropriate.
    • Facilitate evaluation process, complete DRQ, develop the IFSP (if applicable), and provide ongoing service coordination activities to families on caseload.
    • Share IFSP, evaluation, and screening results with families and partnering agencies as requested; within five (5) business days.
  • Maintains required DODD Service Coordinator certification and recertify every two years, or as prescribed by DODD.
    • Participate in training offered by DODD, FCBDD, Central Site, as well as other community based trainings.
Requirements
QUALIFICATIONS:
  • Associate's Degree in a social services field (child development, social worker, education, etc.), experience with home visiting, and a background working with children who have disabilities, special health care needs, and/or delays.
  • Passionate belief in the Y's cause of nurturing the potential of all youth, supporting healthy living for all people and finding ways to help and support our neighbors.
  • Ability to relate effectively to diverse groups of people from all social and economic segments of the community.
  • Knowledge of word processing, spreadsheet and database programs.
  • Must have good time management skills, have good attention to detail, and be competent in Windows/Excel applications.
  • Proven track record of developing authentic relationships with others.
  • Ability to foster a collaborative team approach to solving challenging situations.
