ColemanHealth Services is seeking seeking an individualto provide intensive, case management services to transitional age youth (TAY)individuals with severe and persistent mental health issues in our supportiveliving environments within Portage County, Ohio. The Case Manager will assistwith accessing community based resources, housing, employment, makingappropriate referrals and linking the individual with ongoing mental healthservices. This is a day shiftposition working Mondays-Fridays!

Qualifiedcandidates:

·Three years' experience in mental health orrelated field or BA/BS in mental health or related field

·Have/maintain excellent driving records

·Have or willing to obtain NPI and Ohio Medicaidnumbers

·Experience in case management preferred
