We are seeking an individual to provide care coordination and othercase management services to individuals in Portage County who have mentalhealth and substance use disorders staying on our CSU.

Duties of Case Manager (Crisis Stabilization Unit)include:

• Coordinate ongoing care with internal andexternal healthcare providers
• Assist with symptom monitoring and coping skilldevelopment
• Assist with making healthcare, service, andcommunity resource referrals
• Safety monitoring of clients during waking hours
• Completing necessary documentation related toclient services

Qualifications of Case Manager (CrisisStabilization Unit) include:: 

• BA/BS in mental health/related field or threeyears' related experience
• Have/obtain a NPI number and an Ohio Medicaidnumber
• Have/maintain an excellent driving record

Preference given to the following candidates:

• BA/BS in mental health/related field
• Someone with a CDCA
• Experience in case management
• Experience working with adults in a crisismental health setting
