ColemanHealth Services is seeking seeking an individualto provide intensive, case management services to individuals with severe andpersistent mental health issues in our supportive living environments withinPortage County, Ohio. The Case Manager will assist with accessing communitybased resources, housing, employment, making appropriate referrals and linkingthe individual with ongoing mental health services.

Qualifiedcandidates:

·Three years' experience in mental health orrelated field or BA/BS in mental health or related field

·Have/maintain excellent driving records

·Have or willing to obtain NPI and Ohio Medicaidnumbers

·Experience in case management preferred
