A well-respected Community Hospital in Southeastern Texas has just opened up a great opportunity for a Director Case Management.
This patient-centered, Community Hospital offers a wide range of services including Emergency Care, Surgical Services, Hematology and Oncology, Laboratory, Imaging, and more.
Reporting to the CNO, the Director Case Management will oversee the Case Management Department and will manage a great team of RN Case Managers, Social Workers, and Utilization Review RNs. The Director Case Management will be responsible for supporting the organization’s mission of ensuring comprehensive, quality, and safe patient care. In addition, the Director Case Management will oversee the department budget, care coordination, and discharge planning.
The Director Case Management will work closely with hospital leadership in all departments to ensure patients are receiving appropriate healthcare services both inpatient and once discharged. The Director Case Management will also coordinate patient care and make recommendations to improve patient outcomes. This role will require excellent communication skills as well as the ability to collaborate with Providers and the interdisciplinary team.
