The Role
The Nurse is a key member of the Circulo Health team who cares for the health of our members as a way to ensure the success of the Health Hub.

- Ready to collaborate with members to establish care plan goals, to utilize the most appropriate and current interventions, to work towards achieving the desired outcomes.

- Adaptable, organized, efficient, independent self-starter and problem-solver, a leader, a strategic thinker, and a mentor, who is excited about the big picture of whole community health.
- Flexible in an ambiguous and dynamic environment.
- Ready to manage a panel of members (from infants to older adults) to support the member’s health goals holistically
- Experienced in comprehensive care management, chronic disease management, urgent home-based and community-based primary health visits, preventative care, and wellness.
- Experienced with virtual member engagement, member-centered action and/or care plans, resource management.
- Strong communication and relationship skills with demonstrated success partnering with providers and other team members to support wellness and prevention.
- Adaptable, organized, efficient, independent self-starter and problem-solver, a leader, a strategic thinker, and a mentor, who is excited about the big picture of whole community health.
- Willing to commute 2-3 days to Capital Region location(s).
- Registered Nurse with active licensure in the State of New York
- Minimum 2 years direct patient experience
- CPR certification required
- Outpatient/ primary care experience preferred
- Pediatric experience preferred
- Medical, dental, and vision coverage for quality care for all team members and their dependents
- Vacation: Time off is unlimited and required (minimum of 3 days per quarter)
