Pay Range: $72,000 to $81,000

POSITION SUMMARY:

The Registered Professional Nurse (RN) provides care for patients within his/her scope of practice according to the NY State Office of Professions guidelines, within the primary care out-patient setting. Care to Hometown Health Center patients will be provided without discrimination. The RN is part of the Care Team within the health center structure and may act as the Team Lead under the Direction of the RN Care Coordinator.

ESSENTIAL JOB RESPONSIBILITIES:

• Always maintains a professional demeanor.
• Supports the Mission, Values, strategic goals, and objectives of HHC.
• Daily patient assignments and Provider coverage.
• Performs nursing duties in the primary care/outpatient setting.
• Ensures that care delivered is specific to the age, growth, and development of the patient.
• Provides direct supervision of LPNs and MAs, within the care team.
• Assists the Providers in patient care and procedures according to the Health Center’s protocols.
• Provides patient and family education and follow-up as requested by the Provider and/or RN Care Coordinator.
• Assists with the completion of prescriptions during the visit.
• Prepares exam rooms for patient care, including equipment needed for patient care.
• Takes patient into the exam room and performs the patient intake and assessment.
• Administers medication and immunizations as per RN protocols and standards.
• Performs diagnostic testing and treatments per protocols.
• Performs daily inspection and inventory of emergency supplies, including oxygen tanks, suction machines, and medications.
• Maintains competence in clinical practice.
• Always maintains safety and cleanliness of clinical areas.
• Assures correct sterilization procedures for instruments and equipment in accordance with approved protocols.
• Communicates effectively and appropriately with all personnel.
• Participates in QA/QI activities.
• Educates team about infection control practices.
• Participates in the orientation of nursing staff.
• Ensures that patient educational materials are available to patients.
• Always maintains patient confidentiality.
• Adheres to Nursing Dress code policy.
• Supports additional tasks functions as delegated to by the RN Coordinator in collaboration with Providers, and/or VP of Clinical Services.
• Other duties as assigned by the VP Clinical Operations.
• Telehealth home visits for patients (with no web enablement and/or homebound) with medical and dental care needs.
