Hometown Health Center is Hiring a Quality Manager Near Newport, ME

Hometown Health is looking for a skilled Quality Manager to join our team. Hometown Health Center is a Federally Qualified Health Center located in rural central Maine. We are a leader in healthcare, providing more than 32,000 medical, dental, and behavioral health visits per year to 6,200 patients. HHC is growing with a new state of the art facility currently under construction in Palmyra. The Quality Manager is charged with developing, implementing and providing oversight of quality improvement to ensure delivery of the highest quality patient care, optimal patient flow, provider productivity and continuity of care. The key duties of this position are:

  • Develops a conceptual framework for quality measurement and improvement activities across clinic sites. Summarize findings in the annual Quality Improvement (QI) Plan.
  • Collaborates with CEO, COO, CMO, as well as members of Management to develop and ensure the implementation of policies to minimize risk within the organization.
  • Promotes and oversees quality activities and improve operations that support achieving quality benchmarks to achieve incentive payments.
  • Works closely with CFO to track, develop and implement incentive payment achievement.
  • Promotes the use of technology and clinical decision tools to achieve quality outcomes.
  • Updates the quality improvement plan and quality improvement workplan annually.
  • Leads the organization in review of clinical quality outcomes.
  • Coordinates with the Information Systems Coordinator and OSIS to develop audit systems, perform data analysis and prepare reports related to measuring objectives and tracking outcomes as required by grants, contracts or HHC need. Analyze and report data, as required, in conjunction with the QI Reporting calendar and for continued recognition through NCQA and ACO activities.
  • Coordinates with the COO, CMO, Assistant Medical Officer as well as members of Management to organize and lead chronic disease care improvement efforts.
  • Participates in quality components of CCPM – Accountable Care Organization.
  • Evaluates organizational risk and promotes patient and staff safety.
  • Provides oversight of incident reporting process.
  • Leads review of events requiring root-cause analysis and disseminates lessons learned and process improvement plans.
  • Analyzes reports that identify risk management and patient safety patterns and trends for leadership and develops and implements policies regarding risk management and risk mitigation.
  • Works with CMO, Assistant Medical Officer and CEO to monitor and track organizational performance as it relates to meeting FQHC standards, to include FTCA and grant requirements.
  • In conjunction with the CMO and Assistant Medical Officer develops new or update existing clinical outcome measures, protocols, policies and procedures to ensure compliance with policies through a peer review process.

This position requires a strong level of confidentiality due to the sensitivity of materials and information handled. The successful candidate must have excellent communication and organizational skills and possess the ability to handle multiple priorities while remaining professional and calm. It is essential that the candidate have a clear understanding of clinical quality guidelines, compliance and policy development.

EDUCATION AND EXPERIENCE

  • Health related degree and current licensure, as applicable, in nursing, public health or equivalent.
  • A minimum of three year’s related work experience in quality management, utilization or risk management areas or equivalent combination of education and experience.
