Description

The position is located at Faulkner Place. The schedule for this position is Sunday through Thursday 3:00 pm to 11:30pm, Wednesday which requires a mid-shift 11:00 am to 7:30pm

Under close supervision, the Program Coordinator works as a member of a multi-disciplinary team to implement and monitor treatment for residents within the residential facility while supporting a Trauma Informed Care environment. With the high prevalence of trauma among individuals receiving mental health services, it is required that the Program Coordinator understand the effects of trauma on health, coping, and other aspects of the lives of those we serve. This position involves participation and implementation of program goals and objectives to promote a strengths-based, client centered approach to assisting residents in achieving individual goals and ultimately transitioning to a less restrictive setting.

All incumbents must understand, appreciate and respect the diversity and cultural differences within our Cascadia community. It is expected that there is an awareness and sensitivity to the perspectives and perceptions to those we serve, as well as our colleagues.As Cascadia is Trauma informed, the incumbent must support a Trauma Informed Care Environment. With the high prevalence of trauma among individuals receiving behavioral health services, it is required that the incumbent understand the effects of trauma on health, coping, and other aspects of the lives of those we serve.

Essential Duties

This position description is not intended to be an all-inclusive list of responsibilities or an all inclusive job description.

  • Work closely with Program Manager and Program Clinician to complete Residential Service Plans, Resident Assessments and Plan of Care requests as well as track and monitor Prior Authorizations.
  • Under supervision of Program Manager, coordinate residents’ intake and discharge.
  • Assist in updating annual resident documentation as needed.
  • Model group facilitation skills for other staff members including assisting with the development of groups.
  • Facilitate a minimum of one group per week.
  • Integrate best practices into the day to day operations including: Trauma Informed Care and Motivational Interviewing.
  • Identify and prioritize with the client their treatment plan goals. Continuously assess and measure progress toward treatment goals. Redefine goals throughout treatment in coordination with treatment team.
  • Provide individual(s) or group skills training in areas such as self-care, hygiene, community resources, and other life skills as directed by the plan of care.
  • Assist in meal set-up, preparation, serving, and clean up related to meal times or cooking group activities. Perform other household chores as needed, including but not limited to, light housekeeping and cleaning of biohazards.
  • Accompany residents to, and assists during medical appointments in the community (as needed).
  • Coordinate with oncoming shift regarding resident concerns, building/program needs, and any issues relevant to ensure the smooth operation of the program.
  • Provide guidance, training and support to other staff, as assigned.
  • Assist in de-escalating crisis situations, following emergency protocols and procedures. Engage in physical and mechanical restraint principles as appropriate; demonstrate initiative in provision of alternatives.
  • Administer seclusion and restraint techniques as defined in non-violent physical intervention training.
  • Document and maintain electronic health record (EHR) and ancillary records in a manner that assures compliance. Maintain accurate and up-to-date documentation associated with billing, assessment, treatment, residents’ progress and other activities as required.
  • Comply with all agency policies, program procedures and local, state, and federal regulations.
  • Respect and honor client rights and responsibilities and demonstrate professional boundaries and ethics.
  • Demonstrate responsibility for safety of consumers and/or residents, staff and property; demonstrate familiarity with fire regulations and evacuation procedures.
  • Adhere to mandatory abuse reporting laws and HIPAA requirements.
  • Complete annual employee training requirements on a timely basis as indicated in the licensing requirements for the facility as well as agency requirements. Attend seminars, training and other educational opportunities in order to develop professional skills and abilities.
  • Attend clinical case conferences and staff meetings to gain knowledge and communicate program and resident needs.
  • Participate in all scheduled staff meetings, supervision sessions, and other departmental and agency meetings.
  • Participate in regular supervision.
  • Remain awake on all shifts.
  • Perform other duties as assigned.

Qualifications

  • Must meet the requirements and demonstrate the competencies to be credentialed as a Qualified Mental Health Associate (QMHA). Must have a Bachelor’s Degree in a behavioral science field from an accredited college or university or three years of mental health work experience or a combination of work experience and education. Certification must be current and in compliance with MHACBO
  • Experience working with adults with severe and persistent mental illness
  • Knowledge of Motivational Interviewing and the components of maintaining a Trauma Informed Care environment
  • Understanding of principles of Illness Management and Recovery, and the skills to provide strengths-based services
  • Operate Computer hardware and software (Including Electronic Health Record Software) at a level to effectively perform the job functions, including entering and retrieving data
  • Ability to perform CPR, First Aid, and properly use AED equipment. CPR/ First Aid certifications are required and must remain current. Training is provided

Working Conditions

  • Environmental: Work is performed in a secure (locked) residential setting. The nature of the work dictates that the employee is alert and attentive during his or her assigned shift. Rest breaks and meal periods are subject to interruption. Daily routines may be hectic and will likely require the Program Coordinator to perform various site functions and, on occasion, perform the tasks of other employees as needed. There is moderate to high risk of personal injury/illness due to violent behavior, exposure to disease, biohazards, noise and contaminants.
  • Mental: The Program Coordinator will have a varied routine which requires the ability to address new and usual circumstances in which outcomes may negatively affect costs, employee morale or clients. There is a degree of unpredictability and disruptions, requiring flexible time management
  • Physical Demands: Work may include the use of company provided transportation in order to transport clients. The Program Coordinator must have the ability to assist residents in participating in a variety of active leisure and recreational events, as well as skill building outings and accessing community resources. The Program Coordinator must have the ability to assist the team in providing safety to all clients and staff in potentially volatile situations. When the need arises, staff may be asked to perform non-violent physical interventions, holds, and physical transport of individuals to seclusion rooms, as well as assist in the application of mechanical restraints.
