POIC + RAHS is Hiring a Success Plan Program Coordinator Near Portland, OR

Location: Works in the community and across multiple POIC locations within the Portland metropolitan area.
Hours: Monday - Friday 8:30 am – 4:30 pm.

Reports to:
Director of Youth Services
POIC RAHS is a multi-faceted agency that provides wide-ranging services meant to promote stronger attachments between disenfranchised youth, their families, and the wider community. POIC RAHS supports several school-based programs for students in grades 6-12 located throughout the Portland Metro area. All are accredited schools serving more than 350 youth from Portland Public Schools and East County. All programs include paths to earning a high school diploma and/or a GED. We continue to mentor and support our graduates until they reach the age of stable maturity at age 25.
Job Summary:
The Success Plan Coordinator is responsible for ensuring objectives and deliverables are met according to contract and grant agreements, including but not limited to providing one on one participant meetings, student support services, personal development skills, education intervention services for students (14 to 21) attending RAHS. Core responsibilities include:
Responsibilities and Duties: 
  • Supports students with credit attainment, campus engagement, attendance support, family outreach, needs assessments and Violence Intervention Program implementation.
  • Works with Youth Services Department staff to maintain open and clear communications.
  • Implements and track data collection, and ensure grant reports and proposals are prepared on time and on budget. 
  • Supports a caseload of 30 to 60 RAHS Teen Peer-Mentor Volunteers, which will include needs and risk assessments, Individual Success Plans (ISPs) implementation and tracking, resource referral, as well as recruitment and coordination of program implementation at PPS and East County programming at elementary and middle schools near RAHS campuses.
  • Works with students to connect them with POIC RAHS programs that may include after-school activities, tutoring, employment and training services, parenting education services, mentoring as well as juvenile justice prevention/intervention services and volunteer opportunities. 
  • Collaborates with POIC RAHS colleagues to measure and report student growth in areas such as credit attainment, school engagement, discipline incidents and school attendance where applicable. 
  • Serves as liaison between assigned students, their families, additional support programs, our program and will have a thorough knowledge of the theories, methods and techniques of social casework, strength-based interviewing, adolescent development and will be able to model professionalism and servant leadership.
  • Weighs in on strategic plans for the Department that relate to the future of youth and parent/family involvement programming and community engagement.
  • Ensure budgetary compliance and submit any required documents or reports on time and complete
  • Working knowledge of social service practices and experience working with families from diverse backgrounds. 
  • Attend Bi-monthly staff meetings.
  • Ability to assess risk factors, protective factors, personal and evaluation needs to develop appropriate individual interventions or case plans.

Experience/Education:
  • Experience working in social work, education, criminal justice, psychology, or related human services field. Bachelor’s Degree preferred.
  • 3 years’ experience providing case management services. 
  • Experience providing outreach, crisis intervention, and conflict resolution skills with youth and families.
  • Strong cultural competency.
  • Experience conducting home visits.
  • Ability to pass employment screens, including, but not limited to criminal background, sex offender, drug, abuse, and neglect
Certifications:
This position requires a valid insurable Driver’s License.
