We are looking for an excellent multi-tasker, detailed-oriented, results driven care coordinator for all of our skilled care patients. This person must work well with tight deadlines and creating good relationships with all our referrals . This role is key to the success of the business with the main responsibilities of scheduling our nurses with their client's (home health and private duty nursing) and processing all skilled nursing in-takes from start to finish.
Our Skilled Care Coordinator must thrive in a fast-paced environment, be able to answer phone calls and resolve issues quickly and accurately, be able to motivate and inspire a team and respond effectively to the changing needs of our clients both young and old.
