JFCS is seeking a full-time, Geriatric Care Coordinator, to provide older adults and their family members with help in managing healthcare and daily living needs. The position will function as a guide and advocate, providing support and resources to help seniors manage their living situation, improve quality of life, plan and manage long-term care and navigate the healthcare system. Working knowledge of community resources is essential. Successful candidates will possess strong interpersonal skills, attention to detail and the ability to manage time and prioritize tasks. Background in social work, nursing or related experience preferred.
