Position Overview:

Community Health Workers (CHWs) at Bread of Life play a crucial role in supporting individuals and families by providing free support services aimed atimproving their overall health and wellness. By working closely with community members and identifying non-medical drivers that affect their health, you will help develop personalized health goals and provide assistance in accessing a range of resources including medical care, mental health services, nutrition, clothing, and more. Your work will not only address immediate needs but also contribute to long-term well-being and independence for those you serve.

Responsibilities:

  • Care Coordination: Provide timely, responsive care coordination to community members, ensuring they have access to the resources and services that meet their needs.
  • Community Engagement: Foster relationships with community members and organizations, working to build a network of support and collaboration.
  • Case Management: Efficiently balance caseload needs based on client priorities, managing time effectively to ensure each client receives the attention they require.
  • Documentation: Document all client interactions, progress, and resource connections accurately using our data collection platform, enabling quick mobilization and informed decision-making.
  • Event Participation: Actively participate in community health events, including a mandatory Bread of Life distribution event one Saturday a month.
  • Professional Development: Attend all required meetings, trainings, and special events, demonstrating punctuality and commitment.
  • Communication: Maintain timely and professional responses to all forms of communication, ensuring clear and effective interaction with clients, team members, and community partners.

  • Advocacy: Ability to build trusting relationships with clients, advocating for their needs and empowering them towards independence and long-term success.
  • Cultural Competency: Demonstrate sensitivity and ability to engage with diverse cultural backgrounds, understanding and respecting community member's experiences and needs.
  • Organizational and Time Management: Exceptional organizational skills with the ability to prioritize tasks, manage a varied caseload, and efficiently allocate time responsibilities to meet deadlines.
  • Community Resources Navigation: Knowledge of local resources and services, and the ability to guide individuals in accessing these resources effectively.
  • Conflict Resolution: Managing and resolving disputes or disagreements constructively and maintain positive relationships
  • Collaborative Engagement: Demonstrate the capacity to engage constructively with team members and community partners, fostering an environment of mutual respect and shared purpose to address community health challenges collectively.
  • Flexible Communication Style: Demonstrate the ability to effectively communicate across multiple platforms, including in-person, phone, and digital channels, to meet the diverse needs of the community.
  • Technology Proficiency: Demonstrate strong skills in navigating digital platforms, including data management systems, spreadsheets, and online communication tools for engaging with clients and partners.
  • Bilingual Proficiency (Preferred): Fluent in both Spanish and English, enabling effective communication with a diverse community. Preferred but not required.

Qualifications:

  • Experience (Preferred): Prior lived experience and/or work experience in community health work, social services, or the medical field.
  • Certification (Required): Community Health Worker Certification
  • Reliable Transportation (Required): Ability to independently commute to worksites
