We are looking for a Full-Time Community Health Worker II to join the Center for Health Equity Research as part of the UTHealth School of Public Health. The Community Health Worker is responsible for developing strong relationships with high-risk pregnant patients receiving prenatal care from Harris Health System. The CHW will support recruitment and enrollment into a produce prescription program implemented with academic and community partners over two years. The program aims to assess the feasibility and preliminary effects of differing doses of home delivery and produce a prescription program on a wide range of clinical and non-clinical outcomes among low-income, ethnically diverse pregnant mothers. This is a hybrid (in-office/remote) position.

Position Key Accountabilities:

  • Motivate patients to be active, engaged participants in their health.
  • Help patients navigate resources, including nutrition education resources, and complete study surveys.
  • Assist patients by identifying and addressing their barriers to good health, communicating with other care team members around patient needs, and coordinating care and resource assistance.
  • Coach patients in the effective management of their pregnancy and self-care. Follow-up with health management/care plans and instructions
  • Collaborates and communicates effectively with internal and external partners.
  • Delivers project services according to scope, protocols/procedures, and timelines to effectively achieve stated goals and objectives.
  • Conducts administrative tasks accurately, consistently, and within set deadlines; this includes daily assignments, project tracking, monthly reports, and other tasks necessary to meet stated objectives.
  • Serve as a key point of contact for patients by fostering trusting relationships in conducting health promotion activities with the maternal cohort at Harris Health
  • Effectively work with people from diverse backgrounds in reducing cultural and socio-economic barriers between patients and institutions.
  • Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place.
  • Provide information about health issues and program services to the user population.
  • Continuously expand knowledge and understanding of community needs, resources, services, and programs provided.
  • Instructs individuals and families in health education and disease prevention.
  • Travel to service locations as assigned.
  • Maintain confidentiality with patient information per HIPAA guidelines.
  • Any other related duties as assigned by supervisor/manager.

Certification/Skills:

  • Texas Department of State Health Services (DSHS) Certified CHW
  • Bilingual (English/Spanish) 
  • Must have the ability to perform complex and diverse administrative duties that involve the application of procedures and demonstrate appropriate judgment within the CHW scope of work.

Preferred:

  • Clear and understandable verbal communication and interpersonal skills
  • Detail-oriented and self-motivated
  • Able to listen well, accurately communicate ideas, and to work well in a team.

Minimum Education:

  • High school diploma or equivalent. 

Minimum Experience:

  • Three (3) years of related experience in health, social work or education. May substitute required experience with equivalent years of education beyond the minimum education requirement.

Physical Requirements:

Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently and/or a negligible amount constantly to move objects.

Security Sensitive:

This job class may contain positions that are security sensitive and thereby subject to the provisions of Texas Education Code § 51.215

Residency Requirement:

Employees must permanently reside and work in the State of Texas.
