Vecino Health Centers is Hiring a Bilingual Community Health Worker Near Houston, TX

PURPOSE

Conduct outreach activities, deliver health literacy programming in the community, and provide navigation services to connect community members within the Vecino 25-zip code HRSA Service Area (SA) to services. Additionally, the CHW work collaboratively with Vecino key partners such as the UH Community Health Worker Initiative (CHWI), Bread of Life, the 5th Ward Health Collaborative, the Northside Health Collaborative, school-based behavioral health sites, and other public and nonprofit entities within our SA. The CHW is part of the enabling services team and support other health center activities as needed. It is expected that this position will spend up to 75% of the time in community work including outdoor outreach events engaging diverse members of the community.

ESSENTIAL JOB DUTIES & RESPONSIBILITIES

1. Develop and implement weekly and monthly work plans to engage individuals within the Vecino 25-zip code HRSA SA to conduct outreach activities, provide health literacy workshops, and navigation services.

2. Impart health literacy programming in partnership with public and nonprofit entities, as well as faith-based communities.

3. Conduct asset mapping within the Vecino SA to create intentional plans to increase Vecino’s footprint and develop community partnerships.

4. Recruit new patients.

5. Help recruit participants for Vecino’s health literacy programming.

6. Identify and address barriers to care to ensure the effectiveness of navigation services for members of the community.

7. Document and follow up with community requests for outreach, health literacy, and navigation services.

8. Keeps documentation for tasks and activities assigned by supervisor such as records of outreach events, contacts made, and people engaged in both health literacy and navigation services to issue weekly, monthly, quarterly, and annual reports.

9. Help builds collaborative efforts within the Vecino service area.

10. Participate in trainings and team activities in collaboration with key Vecino partners.

11. Adhere to policies and procedures outlined by Vecino and its core project partners.

12. Be a valuable team member and support the work of other enabling services staff members.

13. Support health centers frontline services during high volume days/seasons.

14. Other duties as assigned by supervisor.

QUALIFICATIONS, EDUCATION, AND EXPERIENCE

1. Community Health Worker certification required

2. Associate degree highly desirable

3. Minimum of one year of customer service experience or community-based work

4. Bilingual English-Spanish must be able to speak, read, and write fluently in both languages

5. Experience participating and organizing community events

6. Excellent organizational and interpersonal skills to establish effective working relationships and engage community members

7. Tactful and respectful

8. Impeccable work ethics

REQUIREMENT

Ability to sit, stand, bend, and stoop for (long) periods of time

Ability to exert up to 20 pounds of force occasionally/frequently

Ability to work in a stressful environment

Ability to respond to emergency/crisis situations

Exposure to noise and weather conditions during outdoor outreach activities

Evenings and weekend work required
