Job Overview:
As a Case Manager at Stonegate Center, you will play a crucial role in the care and support of individuals battling addiction. You will work closely with clients, their families, and a multidisciplinary team to develop and implement personalized treatment plans aimed at achieving sustainable recovery outcomes.

Responsibilities:
- Conduct comprehensive assessments of clients' physical, emotional, and social needs
- Develop individualized treatment plans in collaboration with clients and their support network
- Coordinate and monitor the delivery of services and interventions to ensure continuity of care
- Provide counseling, support, and education to clients and their families
- Facilitate group therapy sessions focusing on addiction recovery and relapse prevention
- Maintain accurate and up-to-date documentation of client progress and treatment plans
- Collaborate with healthcare professionals, community resources, and external agencies to optimize client care
- Participate in case conferences, team meetings, and continuing education activities
- Adhere to ethical and professional standards in all aspects of job performance

Qualifications:
- Bachelor's or Master's degree in Social Work, Counseling, Psychology, or related field
- Certified Addiction Counselor (CAC) or equivalent certification preferred
- Experience working in addiction treatment or related healthcare settings
- Strong understanding of addiction recovery principles and evidence-based practices
- Excellent communication, interpersonal, and organizational skills
- Ability to work effectively in a multidisciplinary team environment
- Compassionate, empathetic, and nonjudgmental approach to client care
- Knowledge of relevant laws, regulations, and ethical guidelines in addiction treatment
