What will you be doing in your role?

• You maintain regular contact with clients to help monitor and track progress in response to services.
• You will assist expectant mothers with resources to overcome barriers to family stability.
• You will work collaboratively with volunteer mentors, other CCFW programs and community partners to ensure client’s resource stability needs are met.
• You will develop and monitor client-led service stability plan.
• You will ensure accurate and timely documentation to meet agency and/or funder requirements.
• You monitor and evaluate client progress and adjust service delivery plans as necessary.
• You will assist in the delivery of parent education classes.
• You will host trainings and/or informational sessions with partner parishes at external sites.
• You cultivate positive relationships with internal and external partners.
• You are familiar with community resources and other supports to provide clients with appropriate referrals.

Are you the next Gabriel Project Case Manager?

• Bachelor’s degree in Social Work or related field.
• Social work or case management work in a related field.
• Bilingual preferred (Spanish).
• Must have reliable transportation, valid driver’s license, and liability insurance.
• Must be willing to use personal vehicle to travel to and from external partner locations.
• Must have ability to work a web-based database.
