A long-standing organization dedicated to providing social services to families, children, and adolescents in the New York City area is currently seeking Bilingual Case Managers for their Bronx location. You would be tasked with managing a caseload of clients, connecting them with needed medical, mental health, and/or substance abuse services. The base pay for this position is $20 to $24 per hour. The actual compensation offered to the successful candidate may vary from posted hiring range based on geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of the offer. *Responsibilities:*
* Manage a caseload of clients.
* Connect clients with needed medical, mental health, and/or substance use services
* Collaborate with other professionals to identify wrap around services for clients in outpatient behavioral health and other social service programs
* Connect clients with services including insurance, housing assistance, educational and vocational training, supported employment, specialty medical care and other social service needs.
* Accompany clients to appointments and perform home visits as needed
* Contribute to case conferences
* Organize group trainings to support clients as they work to integrate into their communities and lead productive, satisfying lives.
*Qualifications:*
* Bachelor's degree in social work, human services, healthcare, or a related discipline
* Minimum of two years experience in the human service field
* Must be bilingual in English and Spanish
