A health services organization in New York City has a great Case Manager position in the Bronx awaiting a compassionate, Spanish-speaking professional. About the Opportunity:
Setting: Residential Group Home
Schedule: Monday to Friday
Hours: 9am to 5pm (1 hr unpaid lunch)
Responsibilities: The Case Manager will:
Ensure that each client is receiving appropriate and effective case management services
Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals
Conduct face to face contacts (home visits/office visits) as specified in the service plans
Review the case management service plans and conduct reassessments on a quarterly basis to ensure that services are appropriate for the clients’ current needs
Provide crisis intervention to clients and their families, as needed
Maintain ongoing awareness of clients’ medical conditions so that services are appropriate to changing needs
Arrange support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services
Provide counseling and orientation regarding tenant’s rights and responsibilities to maintain and retain housing
Assist clients in locating alternative housing options
Maintain and complete required documentation, including all contacts made with the client and any contact on their behalf
Participate in discharge planning with hospital staff for a hospitalized clients
Provide support and education to the client on budgeting to meet financial obligations
Perform other duties, as needed
Qualifications:
6 months of relevant work experience
Bachelor's Degree
Individual, Family and Group Counseling experience
Experience with with Mental Health illness affected populations and/or Substance Use Disorder individuals.
Knowledge of at least one of the following service areas: Homeless-related issues, Criminal Justice, HIV/ AIDS, Substance Abuse, etc.
Extensive knowledge of Case Management principles and practices
Good initiative and learning ability of HRA-HASA Scattered Site Program Desk Guide and the HIV/AIDS Program, Office of Mental Health regulations
Microsoft office proficient
Solid problem solving and time management skills
Great interpersonal skills
Excellent communication skills (written and verbal)
Strong attention to detail
Highly organized
Bilingual Spanish
Desired Skills:
Experience in Scattered Sites Programs
Experience using AWARDs & CARES documentation system
