ESSENTIAL JOB RESPONSIBILITIES:
1.Conduct admission procedure of new residents, (including completing all forms, entering all admission data into computer Assess at the same time their social and “ADL” needs.Complete a social assessment for initial case management in writing on all new residents, and then as events warrant.Complete annual case management as mandated by Regulations.(All these assessments are to be updated daily, and be written in such way, that the individual resident could be readily identified through the notes
2.Meet with residents and families regularly.Work with family members to maintain constructive ties and/or ameliorate conflicts.
3.Help residents make the adjustment to their new home.Introduce new resident to direct care staff, recreation, roommate, and other residents.Give brief history of resident’s past social life, likes and dislikes to staff.Introduce dining room mates, staff and advise staff of any specific diet or food likes and dislikes of the resident.
4.Develop a formal and informal flow of communication and cooperation with all other staff members who serve the resident.
5.Be involved with and provide support to residents andfamilies when transfer to an alternative care setting is necessary.
6.Provide input and assistance to the activity program.
8.Refer and seek the assistance of professional services for residents for their physical and mental wellbeing.
9.Work with Administrator and other Management Staff to ensure that the resident’s social and physical needs are met.
10.Check that residents are receiving the necessary services that meet their needs.
11.Strengthen the informal relationship among residents.
12.Work with other resource people to coordinate resident services, located inside or outside our Community.These may be provided by formal or informal helpers (i.e., staff people, professionals or family members and volunteers).
13.Note and notify the appropriate authority if there is a change inthe resident’s mental or physical status.
14.Develop brief case records on each resident upon admission (initial), and then when incidents warrant, also annually, to provide continuity of care.
15.Arrange for service in areas of income maintenance, health, mental health, and social service.Assist residents as they apply for income and public benefit entitlements.
16.Develop a resource file on services, benefits and entitlements for residents.
17.Visit residents when they are in the hospital.
18.Facilitate the remembering of birthdays, anniversaries, illnesses and deaths with cards..
19.Meet with residents to discuss end-of-life decisions
20.Assist with the resident council.
21.Assist the Administrator as a representative of our Community in the community.Review residents service needs periodically and acts as their spokesperson when necessary.Note complains and assist in their resolution.
22.Carry out other responsibilities as assigned by Administrator orPrincipals.
23.Delegate and supervise all staff members when the administrator is not in our Community.
