Family Support Coordinator

Alternative Schools Network seeks an exceptional, mission driven individual to serve as Program Coordinator for its Critical Health Network Program based in Chicago, Illinois.

The ideal candidate will be a self- starter, organized, mission-driven, and passionate about programs that empower at-risk youth in the Chicagoland area. They will possess a deep commitment to empowering youth as the next generation workforce, demonstrate strong verbal and written communication skills, and thrive in a self-starting, yet team driven environment.

*Full time / Temporary Position (Jan 2024 - June 2024)

About Alternative Schools Network & Critical Health Network Program

The Critical Health Network involves local youth and young adults leveraging their social networks and community connections to initiate outreach to local families. Through outreach, we strive to establish trusting relationships with local families by connecting them to free and low- cost community services and by providing emergency food, rent, and utilities support. The CHN is vital to realizing health equity in Chicago both during and after the COVID-19 pandemic.

Through our extensive experience working with out-of-school youth populations, Alternative Schools Network has learned that the most significant factor in reducing dropouts is a whole-school approach that includes alternative education opportunities which can produce a systemic change in how re-enrolled students approach their education.

Alternative Schools Network believes the essential steps to reducing dropouts and thereby transforming youth, the communities they live in and their futures include:

  • Supporting youth in overcoming challenges that may hinder them from staying in school and graduating.
  • Inspiring youth through transformative curriculum, creating space to incorporate innovative programs for marginalized and at-risk youth.
  • Improving the quality of education and transition services, in terms of both academic and soft skills, to prepare students for college, employment or training.
  • Engaging youth through personalized academic attention and extracurricular activities.
  • Expanding opportunities to which they may not otherwise have had access in order to unlock new interests, develop new skills and gain pivotal social-emotional learning.

Family support coordinator is responsible for connecting families to identify healthcare provider and free or low-income community resources needs. The program goal is intending to reduce health inequities and expand wellness opportunities for the residents of the neighborhoods listed. The program will provide community assistance (connection to key and necessary resources) and health assessment operations, particularly COVID-19 vaccination assessment, through work with enrolled families.

Duties:

  • Coordinating case management services, including managing paperwork and communicating with other professionals involved in the enrolled family case.
  • Meeting with clients individually to discuss their needs and progress over time.
  • Coordinating with other community agencies to plan events for families, such Conduct community events to promote Health Screening & COVID-related resources (testing, PPE kit)
  • Help families access food, clothing, shelter, and other basic needs.
  • Helping families find job training and employment opportunities within their skill set.
  • Conduct a quarterly survey with the families regarding program services.
  • Acting as a liaison between clients and other community agencies, such as schools or medical facilities

Qualifications

  • A High School Diploma
  • Experience working with diverse communities and youth development
  • Excellent time management and organizational skills
  • Highly organized and able to manage multiple competing priorities.

Job Type: Temporary

Pay: $31,000.00 per year
