ESSENTIAL JOB DUTIES

Operational Logistics (15%)

·Develop and implement operational plans for continentals’ quality improvement, ensuring alignment with organizational objectives and regulatory requirements.

·Coordinate with department heads and frontline staff to integrate continuous quality improvement initiatives into daily operations.

·Conduct regular audits and assessments to monitor adherence to quality indicators and identify operational areas for improvement.

Quality Leadership and Collaboration (40%)

·Chair Policy and Quality Committee meetings, facilitating discussions and decision-making processes collaborating with committee members to develop action plans and recommendations for quality improvement initiatives.

·Establish and measure quality indicators periodically per Ql reporting schedule. Review and evaluate patient medical records as needed to assess quality indicators.

·Facilitate regular meetings with department heads and frontline staff to discuss quality improvement initiatives and share best practices.

·Manage operations data analysts, providing guidance, mentorship, and support to ensure their professional growth and development.

Participation in Quality-Related Programs (20%)

·Represent Health Delivery in strategic relationships with payers, grants and other state programs. Direct and participate in the design, development, and implementation of value-based care delivery models to improve quality outcomes.

·Coordinate with department heads and frontline staff to integrate PCMH principles into daily operations, focusing on patient-centered care, care coordination, and population health management.

·Conduct regular audits and assessments to monitor adherence to PCMH standards and identify operational areas for improvement.

·Attend conferences, workshops, and training sessions to stay updated on industry trends and best practices in quality improvement and operational excellence.

·Share knowledge and lessons learned with internal teams to drive continuous improvement and innovation in care delivery processes.

Performance Improvement (15%)

·Apply lean management principles and methodologies to identify and eliminate waste, streamline processes, and improve efficiency in quality improvement initiatives.

·Lead lean management projects aimed at reducing wait times, improving resource utilization, and enhancing overall workflow effectiveness

·Conduct value stream mapping exercises to visualize current processes, identify bottlenecks, and develop future state workflows aligned with quality improvement goals.

·Facilitate Kaizen events and rapid improvement workshops to engage frontline staff in problem-solving and continuous improvement efforts

·Utilize lean tools such as 5S, kanban, and visual management techniques to standardize processes, enhance communication, and promote a culture of continuous improvement.

·Monitor key performance indicators (KPIs) to track progress and measure the impact of Lean initiatives on operational efficiency and quality outcomes

·Provide training and coaching to staff on Lean principles and tools, empowering them to identify improvement opportunities and drive sustainable change at all levels of the organization.

Policy Development and Management (10%)

·Working closely with the Chief Officer of Operations to develop, review, and revise policies and procedures that are approved by the Policy and Quality Committee.

MARGINAL JOB DUTIES

JOB SPECIFICATIONS

1.Education: Bachelor's degree in Public Health, Nursing, or Health

Administration required. Master's degree preferred.

Or, any equivalent combination of experience and training that provides the required knowledge, skills and abilities.

2.Licensure: RN, if applicable.

3.Experiences: Experience in clinical quality improvement (implementing,

monitoring and evaluating quality initiatives) of at least two years. Previous experience in managing teams and/or team processes. Experience in performance improvement methodologies.

4.Skills: Ability to communicate clinical information effectively. Ability

to organize various personnel to achieve desired results. Skilled in written and oral communication. Attention to detail with demonstrated organizational skills and the ability to complete projects on time. Ability to produce, interpret and monitor data for decision-making. Proficiency with Microsoft Office software and/or similar applications.

5.Interpersonal Skills: Able to prioritize tasks on a short and long -term basis. Able

to handle multiple tasks efficiently and effectively. Able to communicate effectively with a diverse population including physicians, mid-level providers, clinical personnel and administrative personnel.

6.Physical Effort: Medium.

7.Hours of Work: Full-time; Flexible and varied.

8.Travel: Travel within Health Delivery service area, some out of town travel

may be required. Reimbursement consistent with GLBHC policy.
