GLBHC is Hiring a Behavioral Health Case Manager (Child/Adolescent) Near Saginaw, MI

JOB SUMMARY

Accomplishes clients’ care by assessing treatment needs. Implements, monitors and evaluates treatment plans and progress. Responsible for facilitating interdisciplinary approach. Participates in a team-based approach to care in the Patient Centered Medical Home (PCMH). Participates in activities related to Quality Improvement.

ESSENTIAL JOB DUTIES

1. Reviews client records, goals, needs and treatment plans.

2. Establish positive case manager relationships with patients and family members involved by building rapport and transparency within the professional relationship.

3. Establish and assist in engaging clients in treatment programs by setting schedules and routines, coordinating services being provided, arranging resources, including transportation and escort and continued care as needed for the patient.

4. Monitors cases by verifying clients’ attendance, advocating for identified services, obtaining additional resources, intervening in crises and providing personal support.

5. Maintains clients’ records by reviewing case notes, logging events and progress.

6. Regularly communicates clients’ progress by conducting interdisciplinary meetings and evaluations, disseminating results and obstacles to therapeutic team and family.

7. Prepares clients’ discharge by reviewing and amplifying discharge plans, coordinating discharge and post-discharge requirements.

8. Advocating and collaborating with all service providers involved in the case and family.

9. Regularly checks in with family/patient via phone to verify stable and consistent living environment.

10. Provides educational support to patients and family to assist them in understanding diagnosis and treatment needs.

11. Assist in any other tasks deemed necessary by the multifunctional team.

MARGINAL JOB DUTIES

1. Serves as liaison between GLBHC and local institutions/organizations to promote behavioral health services as appropriate.

2. Other duties as assigned.

JOB SPECIFICATIONS

1. Education: Bachelor’s degree in Social Work from an accredited educational program.

2. Licensure: Licensed as a LBSW or LLBSW in Michigan.

3. Experience: Minimum of one (1) year of experience providing services to Children and adolescents.

4. Skills: Ability to communicate with diverse populations. Able to coordinate patient services and deal effectively with other service organizations. Skilled in written and oral to demonstrate positive customer service skills.

5. Interpersonal Organized and effective with limited supervision. Ability to provide services in an empathetic, non-judgmental and objective manner.
