Job Summary
This position includes a $2,500 sign-on bonus!
The Care Management Nurse provides care, treatment, counseling, health teaching, and coordination of care to patients who are experiencing changes in the normal health processes or who require assistance in the maintenance of health and the prevention or management of illness, injury, or disability. Collaboratively work with providers on developing treatment plans. The scope of nursing includes the teaching, direction, and supervision of unlicensed personnel in the performance of delegated nursing activities.
Requirements
- Associates degree in Nursing from an accredited nursing program; BSN degree preferred
- Minimum of one year experience as a Registered Nurse with recent clinical experience.
- Current unrestricted State of Michigan RN License
Compensation, Benefits & Structure of the Care Management Nurse Position
This position includes a $2500 sign-on bonus! This position includes a competitive salary structure based on skills and experience and a comprehensive benefit and retirement package.
