OBJECTIVE: To process referrals in a timely manner while assisting with providing supportive services to clients.
JOB REQUIREMENTS:
Build high quality relationships with participants to understand the personal experiences of those impacted by homelessness.
Administer the Youth VI-SPDAT, the standardized housing intake;
Assist with move-ins, move-outs, and housing transitions;
Assist with the "Make Ready" process for unoccupied units
Maintain the Referral Prioritization List
Obtain required intake documents from clients prior to entry (ex: identification documents & homeless verification letter)
Assist clients with obtaining identification documents
Provide supports to clients following goals developed in their service plan. Supports may include assistance with money management, independent living skills, educational monitoring, employment assistance, and other supports that assure success with a healthy lifestyle and independent living. These may include medication monitoring, personal hygiene skills, grocery shopping, housekeeping, preparation of meals, laundry skills, and community integration.
Document daily activities of clients and support provided to clients in case notes using ServicePoint and/or LSNDC. Documentation of activities and training should follow agency guidelines and state licensing/certification requirements.
In conjunction with the Program Director and/or Case Manager, plan and conduct structured activities for participants.
In the event of illness or injury, take appropriate steps to ensure medical attention is received, if necessary. Report all illnesses or injuries to the Program Director.
Complete documentation regarding interactions with clients.
Transport clients to appointments or social events as needed.
Complete critical incident reports as needed. Follow policy for reporting.
Maintain client confidentiality per HIPAA standards.
Develop and maintain positive relationships with agency staff members, outside agencies, and local community.
Attend staff trainings and meets as required by the agency.
Complete any other duty as required by Volunteers of America North Louisiana
MINIMUM EDUCATION, CERTIFICATION AND EXPERIENCE:
Bachelor degree preferred, High school diploma accepted with experience supporting youth
Prior work experience in a program that serves youth age 18-24 required
Experience working with persons with disabilities preferred
Must have current certification in CPR/First Aid
Strong organizational skills
OTHER REQUIREMENTS:
Must have a valid Driver's License with good driving record
Liability insurance that meets minimal requirements
Must have current safety inspection sticker on car
Criminal background will be checked
Must be available to work flexible hours
PHYSICAL REQUIREMENTS:
Must be able to walk, sit, stand, stoop, bend, climb stairs, see, hear, and speak
Must be able to operate a motor vehicle
Must be able to read and write in order to effectively communicate with clients and other staff members
Must be able to lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds
