Hospice Licensed Social Worker

Full Time - Days

**Pay rate up to $27/hour; based on experience**

McGregor - Supporting Seniors in need and those who serve them.

Our Part Time Hospice Social Worker will work on campus to visit Hospice patients 1-2 times per month or as needed to meet their needs. This role work closely with our chaplain and Nurses.


What do I need as a Social Worker?

  • A graduate of a Master's program in Social Work (MSW) accredited by the Council on Social Work Education (CSWE), with one (1) year experience preferred. Minimum qualification is Bachelor's Degree in Social Work accredited by the Council for Higher Education Accreditation (CHEA) and one year experience.
  • Licensed in the State of Ohio as Licensed Independent Social Worker (LISW) or Licensed Social Worker (LSW) required.

What will I do as a Social Worker?

  • Performs initial screening and evaluation of the psychosocial, emotional, and spiritual status of patients and families/caregivers related to the patient's terminal illness and environment. Engages patient and families/caregivers in the development and implementation of goal-directed IDG plan of care.
  • Carries out social evaluations and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in relationship to issues of death and dying, stress, bereavement, and other identified coping difficulties.
  • Provides short term crisis intervention and individual or family counseling when indicated. Utilizes de-escalation techniques as appropriate. Required to be on-call and available twenty-four (24) hours via telephone or in-person if emergency visit is needed.
  • Assumes an active role of advocate for the patient/family unit.
  • Assists patient and family/caregivers with securing Advanced Directives, Durable Power of Attorney and funeral arrangements, as needed.
  • Assist patients and families in communicating with one another and to members of health care team.
  • Actively participates in the IDG meetings by presenting information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and/or family.
  • Educates residents on the roles of health care interdisciplinary team members
  • Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals as appropriate.
  • Provides information and referral services for patients and families/caregivers regarding practical and environmental needs.
  • Maintains collaborative relationships with organization personnel to support patient care. Promotes communication and collaboration among health care team members. Represents hospice team at facility care conferences.
  • Assist other team members in understanding significant social and emotional factors related to health problems and issues related to death and dying.
  • Attends staff meetings and other meetings as assigned and appropriate. Participates in QAPI processes and projects as appropriate.
  • Actively participates in the discharge process assuring patient and family have resources identified to meet care needs post discharge.
  • Documents assessments, visits, telephone contacts, referral actions and social service interventions in the medical record clearly and concisely in a timely manner.
  • Assists Hospice Chaplain, as needed, in meeting the bereavement needs of patients and their families.
