APN Healthcare Solutions is urgently hiring multiple *Licensed Practical Nurse – LTC *for our client in *Portland, OR *for a 12/13 week contract with a high possibility of extension.
Position Title: *Licensed Practical Nurse – LTC*
Pay Rate: $40 - $42/Hourly
Work Location: Portland, OR
Shift: Night 3x12-Hour (19:00 - 07:00)
Hours/day: 12 Hours
Contract Duration: 13 Weeks
Licenses Requirement: LPN State License
Certification Requirement: BLS
*Job Description:*
* · Administer medications and treatments according to physician orders and facility protocols.
* · Monitor residents' health status and report any changes to the registered nurse or physician.
* · Assist residents with activities of daily living, including bathing, dressing, and toileting.
* · Collaborate with the interdisciplinary team to develop and implement individualized care plans for residents.
* · Provide emotional support and companionship to residents, promoting their overall well-being.
* · Document all nursing care provided and communicate effectively with team members.
* · Ensure a safe and clean environment for residents, adhering to infection control protocols.
* · Participate in resident care conferences and family meetings as required.
* · Respond to emergencies and provide appropriate interventions as needed.
* · Maintain licensure and pursue ongoing education to enhance nursing knowledge and skills.
