$5,000 Sign on Bonus for our CCBHC (Certified Community Behavorial Health Clinic) Outpatient Clinician
Join Sweetser as part of a supportive, integrated team as an Outpatient Clinician! Work with a diverse caseload which is recovery focused and integrated with other members of the patients mental and physical health care team.
ESSENTIAL FUNCTIONS:
* Provide customer service within established deadlines, consistent with organizational values, standards, and policies.
* Demonstrate leadership in Case-Coordination.
* Complete timely, professionally written, documentation.
* Manage insurance authorizations in a fiscally responsible manner.
* Use quality Collaborative Documentation to maintain insurance required documentation.
* Coordinate clinical appointments.
* Use the Organization's established tools and procedures, to formulate a Comprehensive Assessment.This includes, but is not limited to, the Level of Care and Risk, Assessment, involving family members, providers and natural supports.
* Create clinical interventions in an Individualized Service Treatment Plan, reflective of industry standards of best-practice and Evidenced-Based Treatment, which accurately demonstrates that the client's presenting symptoms. Clinician will demonstrate that the prescribed intervention, forms measurable functioning outcomes.
* Demonstrate Trauma-Informed Treatment.
* Provide Integrated Care by including the client's Natural Supports and Providers.
* Provide a clinical experience for student interns and conditionally licensed therapists, as appropriate.
GENERAL EXPECTATIONS:
* Be commited to the mission, vision, and values of the organization.
* Work collaboratively as a member of a team with various groups of staff, depending on the issue addressed.
* Assure quality in work performed to facilitate the delivery of quality services.
* Reports accidents and injuries in a timely fashion, demonstrates safe housekeeping practices, notifies supervisor of potential health and safety concerns immediately, participates in health and safety training as required, understands and follows all Sweetser health and safety policies.
* Ability to support and recognize the important role volunteers play. Sweetser employees work with, support, collaborate with volunteers, and promote opportunities for volunteer engagement.
* Assure quality service for clients by enforcing rules, regulations, and legal requirements with clients.
* Adhere to professional code of ethics.
* Maintain organization credibility by establishing working relationships with sponsoring advisory and related service agencies.
* Promote the organization by understanding services available through Sweetser and informing clients and community representatives of these services.
* Maintain historical records by documenting treatment events and ensuring the timely completion of all programmatic paperwork.
* Contribute to the clinical profession by actively participating in meetings, conferences, and internal trainings; consulting with other professionals; and advocating professional values.
* Contribute to team effort by working with others to meet expected service outcomes.
* Fulfill job function by participating as an active member in all training and participating on organization task forces.
* Perform all other duties as assigned.
EDUCATION:
* Master's degree in Social Work or Counseling, with a concentration in clinical practice from an accredited graduate school. Other specific training or experience may be required based on the community needs at the time of each specific hiring episode.
CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
* Appropriate licensing certification (LCSW, LCPC) preferred.
