Downers Grove Rehab & Nursing is seeking an Admissions Coordinator for our skilled nursing facility in Downers Grover, IL
We are seeking an experienced, highly motivated and dynamic individual to oversee, develop and maintain relationships with local hospitals, physicians and the community to drive patient referral efforts.
QUALIFICATIONS:
Education: At least a high school diploma, two or more years of college preferred.
Experience: 2 years of prior SNF experience Healthcare, human services, public relations, LTC, social services.
Admissions Process Responsibilities
• Manage incoming inquiries, depending on the sales model (i.e., central intake).
• Use the clinical inventory to evaluate residents for admission; when necessary, visits prospective residents in the hospital and make an assessment regarding his/her suitability for admission in the center, when required; Contact Social Services Department to verify eligibility; Contacts insurance companies to determine or verify coverage; Meets in the center with families upon or before admission to sign the paperwork, timely and accurately; Records information that identifies physician and resident, type of accommodation required, insurance coverage, and date of admission, inspects resident’s room before admission, active role in the morning kick-off meeting.
• Arranges room accommodation changes; Give families a list of all department heads and a resident handbook; and interpret the facilities regulations to the resident concerning visitors, visiting hours, and disposition of clothing and valuables.
• Maintain thorough knowledge of the center's products, services, capabilities, physician relations, and payer sources.
Customer Satisfaction Responsibilities
• May assist Administrator in contacting families after admission and discharge to determine satisfaction personally;
• Provides timely follow-up on family or guarantor complaints and problem-solves with residents and/or staff.
• Ensures a Greeter Club/First Impression Program is in place and functioning. Inspects resident’s room before admission; Notifies particular wing to expect resident and arranges for an escort to the room.
Administrative Responsibilities
• Assist the Administrator in developing and training the Admission team.
• Reports observations concerning structural, equipment, and furniture defects and malfunctioning to appropriate personnel.
• At the direction of the Business Development Director, may be asked to attend and participate in staff meetings, departmental head meetings, or other center meetings.
Salary/ Wage Range
Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience and may fall outside of the range shown.
