Vivo HealthStaff is Hiring a Medical Director Near Phoenix, AZ

We are looking for a Medical Director , who will be the clinical leader of our expansion into Arizona as a third market. We are seeking someone who is passionate about the opportunity to redesign the way older adults receive care and ready to take on the opportunity to grow an entire region. This role will oversee primary care providers, provide direct clinical care, and provide clinical guidance to our Care Navigators on a day to day basis.
What you will do:
  • Oversee a team of 6-8 Primary Care Providers (PCPs) made up of physicians, nurse practitioners, and physician assistants to whom you will provide mentorship, education, and support.
  • Interview and hire MDs, NPs, PAs, and MAs to the care team
  • Work directly with the Chief Medical Officer on staffing and scaling within the region and ensure all communities have a clearly assigned PCP
  • Accountable for the clinical and service delivery outcomes of your team including clinical quality, documentation quality and diagnostic capture, patient/family & community satisfaction, and overall team satisfaction.
  • Start by seeing patients directly to learn the care model and transition into spending approximately one half day per week in each provider's panel shadowing and "reverse shadowing" clinicians to ensure standardization of evidence-based practices and educate on how to care for patients in a value-based care model
  • Participate in and sometimes lead root cause analyses of clinical events including errors, near misses, or poor patient or team experience
  • Ensure the development of detailed person-centered care plans on all primary care patients
  • Work with Care Navigators (CNs) to ensure that monthly Chronic Care Management (CCM) visits are completed and reviewed
  • Run regular huddles and provide leadership to the entire clinical team
  • Identify ways of working with senior living management and care teams to better carry out Pine Park care plans and improve patient outcomes
  • Help support both the PCPs and the Care Navigators in creating work-life balance and high work satisfaction
What we're looking for:
  • Board-Certified Physicians in Internal Medicine or Family Medicinepreferably with geriatrics or palliative care experience.
  • Vision, motivation, and excitement for the idea of changing the way healthcare is provided to older adults
  • Comfortable with a fast-paced and changing environment
  • Willingness to provide and oversee onsite clinical care across the San Diego region
  • Desire to work with complex care patients and support the building of a holistic care model that honors patient wishes and reduces utilization of unwanted services
  • Strong orientation towards collaboration and working closely with a multidisciplinary team; values input of all members of the clinical team
  • Strong written and verbal communicator
  • Ability to work with tech-enabled care delivery tools (e.g.,project management, 'checklist' management, remote monitoring, etc.) used together to improve patient care and team function
