Innovative services offered across Chicagoland and Central Illinois include community living options, novel day programs, behavioral health services, employment programs, respite for families, and foster care for children with disabilities.

Purpose:

Member of the interdisciplinary team providing case management and clinical services to persons with mental illness, who may also have a secondary diagnosis of Substance Abuse or ID/DD. Clients may also present with a variety of physical challenges. Must maintain compliance of all clinical documentation for assigned clients. Must attain established productivity targets and complete all clinical documentation in a timely manner.

Essential Responsibilities , include but are not limited to:

  • Conduct assessments, including IM-CANs, to determine need/appropriateness for services, and coordinate linkage to services identified. Responsible for initial IM-CAN completion, as well as updates, as needed.
  • Provide clients with referral, linkage, and follow-up services for psychiatry and others as needed
  • Maintain communication with client’s medical and benefits case manager regarding client medical benefits/insurance, SNAP status/approval, spend-down, and SSI review forms, and transportation services as needed, to ensure benefits and entitlements remain active.
  • Maintain caseload of individual clients, providing individual therapy, in the client’s home, school or community, or MH center contingent upon clients need and preferences. Design and facilitating therapy groups as assigned.
  • Responsible for all clinical documentation for primary caseload including, but not limited to, IM-CANs, LOCUS, SLOF, progress notes and monthly summaries, and consents prior to expiration dates.
  • Provide community support individual and/or group skill building activities as needed.
  • Maintain and update client’s pertinent information within the MH electronic care system i.e. face sheets, contacts, consents, and diagnoses.
  • Achieve and maintain required productivity targets and complete documentation in a timely manner, including clinical documentation, service reporting, and billing.
  • In conjunction with the client’s treatment team, keep client’s family, guardian, and other Envision Unlimited staff informed of the client’s progress, including emergency room visits, hospitalizations, and medication changes/adjustments.
  • Provide information to outside agencies as requested to coordinate and assure the appropriateness of services.
  • Provide and document crisis interventions as needed, within 24 hours of an incident, and complete an incident report when necessary.
  • Other duties as assigned.
Requirements:

EDUCATION, SKILLS, and/or EXPERIENCE:

Master’s degree in social work, counseling, psychology or related field with at least three years’ experience in social services. Experience with seniors preferred. Must have excellent written and verbal communication skills and excellent decision making, problem solving, computer and organizational skills. Individual must be able to work autonomously.

Must have a valid driver's license and own insured vehicle to meet members/clients at different program facilities throughout the city and pass a criminal background check.
