Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance use disorder, as well as an array of other related problems. Since 1969, FGC’s comprehensive programming has educated and guided individuals toward their full potential as productive members within their own families and our society.

FGC’s adult and adolescent programming provides a flexible continuum of care to best address the individual needs of each participant. Through integrated and comprehensive services, including continuous assessment, participants are able to access levels of care and specific services to create an individualized treatment approach. Further, FGC provides special and high-risk populations priority access to specialized programming.

Essential Duties:

Mental Health Assessment

1. Schedule and complete of initial Mental Health Assessment for all mental health clients. This Mental Health Assessment shall be reviewed and approved by an LPHA.
2. Complete The Level Of Care Utilization System (LOCUS) functional assessment for all mental health clients 18 years of age and older.
3. Complete the Columbia Impairment Scale and Ohio Scales for Youth for all clients between the age of 5 and 17 years.
4. Complete the Devereux Early Childhood Assessment Clinical Form (DECA-C) for all children 0 through 5 years of age.
5. Document Admission Note summarizing the clients’ identifying information, presenting problem, history of present problem, provisional diagnosis, safety precautions, the medical necessity of treatment and treatment plan recommendations.
6. Re-assess per Medicaid Title 132 guidelines for ongoing patients.

Treatment Planning

1. Construct and document a timely individualized treatment plan (ITP) in conjunction with the client, and their guardian, in the event they have a court-appointed guardian or are a minor, addressing the clinical needs identified in the assessment process, coordination of care and discharge planning needs. This ITP shall be reviewed and approved by an LPHA.
2. Revise and modify the ITP as the client moves through the treatment process timely.

Mental Health Therapy and Case Management

1. Provision of individual, group, couples/family counseling as driven by the needs identified in the assessment process and as indicated in the individualized treatment plan.
2. Document timely therapy in progress notes summarizing for each session; goals and objectives addressed, clinical interventions, client response to interventions, and continued therapy plan.
3. Provision of mental health case management to facilitate linkage to and ongoing coordination and support for needed services and supports in patients’ community environment.
4. Crisis intervention when indicated including immediate coordination of indicated service given nature of crisis addressed.
5. Engage clients including prompt follow up on attendance issues or barriers, on use of referrals made, etc.
6. Collaborate with the multi-disciplinary team to support integrated care in regard to treatment process.
7. Spend at least 50% of work time providing direct billable services.
8. Develop a working knowledge of FGC applicable Standard Operating Procedures (SOP). 

Work Schedule: Monday - Friday (7:00a - 3:30p) with flexibility to accommodate agency's needs

Pay Rate: $43,860 - $46,808

Education and/or Experience: Bachelor’s degree in associated field required. Master’s degree in Psychology, Social Work, Counseling, or a related discipline from an accredited institution preferred. The Mental Health Therapist is a Qualified Mental Health Professional [QMHP] who has the education, skills and experience to conduct a clinical assessment, develop a plan of treatment and provide treatment for a person with mental illness, under the supervision of a Licensed Practitioner of the Healing Arts [LPHA].

Certificates, Licenses, Registrations: State of Illinois licensure preferred but not required (i.e. Licensed Professional Counselor, Licensed Clinical Professional Counselor, Licensed Social Worker or Licensed Clinical Social Worker). In addition to the above licensure, IAODAPCA Certified Alcohol and Other Drugs Counselor certification or proven experience with substance use treatment preferred. Clearances from CFS 689 Child Abuse and Neglect Tracking System, and CFS 708-C Background Check for Non-Licensed Contract Staff.
